Job summary
Working as part of the provider collaborative the post holder will provide case manager support for a designated caseload of service users within inpatient units across the South West region and out of area. This will include robust oversight of access assessments, ensuring that each service user is placed in the least restrictive setting with the appropriate level of supportive interventions as close to home as possible. As a Case Manager the post holder will work with the pathway managers, ensuring patients have goal orientated treatment and follow appropriate care pathway to discharge. As a Case Manager the post holder will be supported by senior colleagues, with a reduced caseload and no requirement to be on call. The role will support the services in driving transformation as well as value for money in planning, commissioning and service delivery. The role is designed to use a combination of clinical expertise and technical skills to map future service use, identifying and addressing pathway barriers that prevent the repatriation of service users out of area and identifying and addressing challenges that prevent timely discharge or transfer of service users. The post has a critical function in assessing clinical quality issues and identifying any improvements required.
Main duties of the job
As a Band 7 Case Manager, the post holder will work as part of a dynamic team to deliver a service that ensures safe and effective care pathways are in place for service users of specialised mental health care settings within the NHS and the Independent sector. The post holder will be required to support clinical staff and managers across the South West Provider Collaborative footprint. Caseloads vary but will include working with patients within our Adult Secure Services; Adult Eating Disorders, CAMHS Tier 4 and Perinatal.
This role will carry a smaller caseload and be offered increased supervision than the Band 8a Case Managers. Working as part of a busy team the role includes assisting in access assessment quality reviews, ensuring referrals and admissions are managed and appropriate, and that the quality of patient safety, care and treatment is paramount. The successful applicant will be able to demonstrate high levels of skill working with complex individuals, their families and carers within a variety of care settings.
