Job summary
We are looking for a Support Time & Recovery Worker to join the Devon Reconnect Team.
The role is dynamic and challenging. The post-holder will work with individuals pre and post release from prison to provide support with practical concerns or difficulties that might be affecting their mental health and wellbeing, to ensure the continuity of care of an individual from prison into the community.
You will work alongside practitioners in prison, police custody, courts and probation, but mainly in community settings.
The post holder will be expected to work with adults, and young people who have come into contact with the Criminal Justice System with social problems, cognitive disorders, substance misuse problems, mental health problems, learning disabilities and other associated vulnerabilities.
Working as part of Devon Partnership NHS Trust, you will make a difference to peoples lives from the moment you start.
We expect all our staff to embrace this way of working, recognising the equal value of everyone involved in service design and delivery.
The base location of the role will depend on the location of the successful applicant but essentially, this role covers a Devon-wide service.
Main duties of the job
You will be required to work under pressure, without direct supervision and so we are looking for someone confident and capable, with experience of working in the community with a caseload. You will prioritise tasks and manage your own workload, providing support for your colleagues where required.
You will work as part of a multi-agency team, working alongside Criminal Justice Professionals and qualified Senior Mental Health Practitioners as part of the Devon Reconnect Team. Contribute to effective mental and physical health assessments, treatment and targeted interventions for people being released from prison back into the community.
You will actively engage and work with a small number of individuals with offending histories and a wide range of mental health and social problems who may also have co-existing complex needs including homelessness, substance misuse, significant risk to self or others, socially exclusion and who at times may present with extremely challenging behaviours.
Ensuring that individuals engage with services they are referred into, by supporting them to attend appointments. Provide time limited 1:1 support to clients giving advice, information and support to access a wide range of services that provide support with issues such as housing, social isolation/exclusion, debt management, training, education and employment.
Support people who typically find it hard to access community services and resources. Advocate and liaise with relevant community services.
