The South and West Rehabilitation and Recovery Team are looking for a skilled and highly motivated Senior Mental Health Practitioners. The right candidate will have a passion for working with people with complex psychosis, supporting them to reach their recovery goals.
You will be an experienced mental health professional (Registered Nurse, Social Worker or Occupational Therapist) who has a genuine interest in collaborative working with people with complex psychosis, including service users, their carers and families. The role will involve working as part of a wider multi-disciplinary team (MDT) supporting people through the Rehab pathway in both inpatient and community settings. Using your expert skills and knowledge in assessment and support planning. The successful applicant will be innovative and compassionate clinicians, who is motivated to deliver individualised, person-centred care and make a positive contribution to service development
The job covers a wide geographical area covering the whole of South and West Devon so the ability to travel to meet the demands of the role will be essential.
Main duties of the job
With training and support, you will feel confident in delivering therapeutic interventions and liaising with other professionals regarding progress and outcomes.
As a Rehab coordinator you will be responsible for your own caseload and provide assessment, case management and deliver interventions for people with a wide range of needs. You will ensure that the service users recovery goals are central to their Care Plan and promote social inclusion, independence and wellbeing. You will demonstrate excellent communication and team working skills and be able to work collaboratively with a range of statutory and third sector agencies.
You will have an enthusiastic approach towards your professional development.
