Job summary
As a Case
Manager in the South West Provider Collaborative the post holder will work as
part of a dynamic team in delivering an effective service to ensure safe and
effective care pathways are in place for service users of specialised health
care settings within the NHS and Independent sectors. The post holder will be
required to support services across the South West from Cornwall to Dorset to
Gloucester, with additional visits to out of region patients.
Working
across services, the post holder will provide case management direction for a
designated caseload to monitor ongoing therapeutic interventions and pathway
oversight in specialised health care. The role includes robust oversight of the
access assessment process, whilst
ensuring each service user is placed in
the least restrictive setting, as close to home as possible, with goal
orientated treatment for discharge pathway.
The role is
central in supporting the business and driving transformation as well as value
for money in planning, commissioning and service delivery.

against national specifications

with visits to services and patients

oversight of services providing valuable link between commissioner and provider

effectively in a flexible and demanding environment and proactively engage with

- Registered Mental Health / Learning Disability Nurse or other registered health or social care professional Educated to masters level or equivalent level of experience of working at a senior level in specialist area.

- Evidence of post qualifying and continuing professional development. Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement. Must have an appreciation of the relationship between the Department of Health and individual provider and commissioning organisations. Hold current accreditation with relevant professional body.
Knowledge and Skills

- Detailed knowledge of legal issues pertaining to the specialist service, including MHA 1983, after care under supervision, sex offender legislation, probation orders and life licence, Ministry of Justice procedures and child protection legislation. Extensive knowledge of one or more specialist areas, acquired through post graduate diploma or equivalent experience or training, plus further specialist knowledge or experience to masters level equivalent. Such specialist areas will be: Adult Secure - Mental Health / Learning Disability / ABI Child and Adolescent Mental Health Services (CAMHS) including secure Eating Disorders (Adults/CAMHS) Deaf Services Gender Services Extensive knowledge and experience of risk assessment and risk management. Development and maintenance of therapeutic alliance with service users and their families /cares. High-level management skills, able to perform to high level in clinical setting, holding services to account and ensuring effective use of NHSCB resources. Case management skills, focussing on the needs of the individual and the context of their pathway in relation to all clinical and legal processes. Must be able to provide and receive highly complex, sensitive or contentious information; negotiate with senior stakeholders on difficult and controversial issues, and present complex and sensitive information to large and influential groups.
