Job summary

We are looking for an experienced Social Worker to join our team in the Additional Support Unit (ASU) in Exeter.

ASU is a 5 bedded unit for people with a Learning Disability who require assessment and treatment.

We have exciting news that we are in the process of building a new unit with 10 beds for people with a Learning Disability and Autism. The unit is situated on the Langdon hospital site in Dawlish.

Main duties of the job

To be an active member with the inpatient multidisciplinary team, gathering information from service users, informal networks and other agencies to analyse, summarise and evaluate the information to provide a holistic assessment of needs, balancing risk and protective factors a defined caseload of adults of working age, and their families/carers, who have learning disabilities and / or autism, and a co-morbid mental disorder.

Liaising with all relevant parties at the point of admission and maintaining this liaison function through to discharge stage.Complete comprehensive social care assessments and reports as required from the pre-admission to discharge stage, formulating and adjusting of plans.

Arranging Multi Professional Disciplinary meetings as appropriate, also with outside agencies as appropriate, to secure a successful discharge.Collaborating with all relevant parties, including patients and families to support them in the decision making/care planning process.

Contribute to the preparation of Social Circumstances reports for Mental Health Act (MHA) Tribunals, MHA Independent Hospital Managers hearings, and attending these, as required. undertake, and lead on, MCA assessments and BI meetings, DoLS applications, and CoP applications as required.

To lead on co-ordination and monitoring of safeguarding matters for the unit, in liaison with the DPT Safeguarding team.

  • Contribute to the teams engagement with a wide range of stakeholders including staff, service users, their carers and other statutory and voluntary sector agencies.
  • Impart complex and sensitive information to patients and families/carers re assessment, care planning, treatment and review.
  • Multi-disciplinary teams to ensure the provision of consistent well-coordinated care. External agencies also involved in the provision of care eg: voluntary and private sector, police, social services, housing, employment etc. Service user/carer groups.
  • Liaise with a wide range of other professionals and agencies.
  • Uses a high level of communication and interpersonal skills to aid overcoming barriers to acceptance, such as would be required when communicating in a hostile antagonistic or highly emotive atmosphere. Present information some of which may be contentious to patients and their families/carers.
  • To take part in formulation and clinical review of care and treatment plans.
  • Making judgments about complex facts or situations requiring analysis, interpretation, comparisons of a range of options.To work with the clinical team lead in making a judgement in patient management matters.
  • Responsible for developing care packages including assessing, planning, intervention and evaluation. Ensuring active involvement of patients, families and carers.
  • Undertaking assessments regarding current social circumstances and the impact of social issues on patients mental health and recovery
  • Liaising with a host of external agencies to enable them to fulfil their statutory duties, for example: Safeguarding, MAPPA, Local Authority Social Services, Police and Probation.
  • Supporting patients in developing feasible plans for the future and working towards discharge.
  • Supporting carers and families of detained patients.
  • Taking a lead role within the multi-disciplinary teams on non-medical mental capacity issues and best interest decisions.
  • Ensure effective care pathways are in place for transition between mental health and other services.
  • Promote, develop and role model high standards of clinical care.
  • To ensure that robust, up to date risk assessments are carried out by the relevant staff.
  • To ensure standards of care are maintained to the highest level.
  • To promote good practice in the management of patient risk management and safety.
  • Promote a duty of care to users of services; encompassing safe and competent care
  • Peoples wishes, feelings, values and beliefs should be taken into consideration alongside the views of those close to them.

  • A Recognised Social Work Qualification to post graduate diploma level, e.g. CQSW, CSS, DipSW, Degree in Social Work, or equivalent practical experience.
  • Training to level 3 in Safeguarding children and young people: roles and competencies for Health Care staff.

  • Evidence of commitment to life-long learning and continuing professional development.
  • L4 training - Chairing Safeguarding Enquiries.

Experience
