Bristol Dementia Wellbeing Service is a joint venture between Alzheimers Society and Devon Partnership NHS Trust, providing a holistic service for Bristol. This is an award winning, innovative service that is located across two sites in Bristol and supports people with Dementia to live as well as possible, for as long as possible, in their own community.

We have an opportunity for an experienced Community Team Manager who will be accountable for the day to day operational delivery of the service. The role supports and ensure the team engages proactively and positively with the local community both raising awareness of dementia and in providing excellent post diagnostic support.

The post currently available will be supporting our South Bristol team but post will be flexible to work across the city of Bristol as required.

This service provides all staff with opportunities for excellent training and development, supervision and appraisal and not least to make a significant positive difference to the lives of people living with dementia.

Main duties of the job

Community Team Managers Job Purpose includes:

  • Day to day accountability to deliver Dementia Wellbeing Services from one of the community sites
  • Leading the teams engagement in, and liaising with, the local community to deliver great dementia services
  • Work closely with local health and social care partners to provide people with locally based care that supports them and their carers to live well with dementia

Job Purpose

This role has operational accountability to deliver Dementia Wellbeing Services, as part of the Bristol Dementia Partnership, to support people with a diagnosis of dementia to remain as well as possible, for as long as possible, in their own community.

The post holder will provide line management of services to ensure rapid access to credible specialist dementia expertise for GPs, Care Homes, Primary Care staff and others who are requiring support and expert advice on meeting the needs of those persons with a diagnosis of dementia.

Operating from one of two community venues the role will provide effective professional and managerial leadership through appropriate modelling of behaviours and skills and/or direct supervision. They will deliver against a set of key performance indicators and ensure the quality and safety of service provision. They will have the people using the service at the forefront of their decision making and will support staff to deliver safe and effective services.

They will ensure staff work as a team and that the service is responsive, flexible and proactive in meeting the needs of the people using it. Both staff and user feedback is positive and the local community develops a confidence in the service and has a greater understanding of supporting people to live well with dementia.

This role will necessitate working extended hours where needed and will be flexible to work across the city of Bristol as required.

  • Extensive experience of managing a busy caseload of clients with a range of mental health problems.
  • Experience of working at a senior clinical level.
  • Working within a community mental health setting in either the statutory or voluntary sector.

Desirable

  • Budget management.

Qualifications

  • A professional qualification e.g. RN, CQSW/ASW, BSc/Diploma in OT, SROT, Chartered Clinical Psychologist, Physiotherapist

Desirable

  • Post graduate qualification relevant to specialism
  • Management qualification
  • Safeguarding training to the required level

  • An excellent understanding of mental health legislation
  • Awareness of community structures and social care options and processes
  • Excellent communication skills
