Job summary

We have an exciting opportunity for a Case Manager to join two services within the nationally award winning team at the South West Provider Collaborative (SWPC) - Children and Adolescent Mental Health Services (CAMHS) and Adult Eating Disorders (AED).

An opportunity has arisen for a motivated, skilled and knowledgeable practitioner to join the existing CAMHS & AED case management team in the South West Region. The post will be required to work within a specific geographical area of the region. The successful post holder will have case management responsibility for in-region service users and will operate supporting senior members of the team with clinical demands.

The aim of the case management team is to ensure safe and effective care pathways are in placefor all service users requiring CAMHS & AED services in the South West Region within the NHS and Independent sectors.

The successful applicant will be able to demonstrate high levels of skill and knowledge working with complex individuals, their families and carers within a variety of care settings. A understanding of current policy and evidence-based approaches will be critical with the successful applicant demonstrating high levels of knowledge regarding the current CAMHS & AED landscape. Knowledge must include a robust understanding of approaches to managing complex populations in both inpatient and community settings including knowledge of treatment pathways for the cohort.

Main duties of the job

Working as part of the mental health team responsible for the provision of care for specialised mental health, the post holder will provide clinical support for a designated caseload to enable the ongoing therapeutic interventions of service users in specialised secure mental health care. The aim of this clinical support is to have oversight of robust access assessments, ensure that each service user is placed in the least restrictive setting with the appropriate level of supportive interventions to enable their eventual discharge into the community where this is a safe option or maintain their individual needs if they remain in an inpatient setting.

The role will support the business in driving transformation as well as value for money in planning, commissioning and service delivery. The role is designed to use a combination of subject matter expertise and technical skills to project mapping of future service use, identifying and addressing pathway barriers that prevent the repatriation of service users out of area and identifying and addressing challenges that prevent timely discharge of service users from a secure environment.

The post holder will be responsible for supporting specific projects identified by the Lead Provider to address specific priority areas within specialized CAMHS & AED mental health such as the development of the clinical model for both service lines.
