The
Devon CAMHS Assertive Outreach is an innovative, flexible, multi-disciplinary team
offering a dedicated intensive, wrap around intervention to support and
supplement a core CAMHS clinical intervention plan. This enables us to support children with
severe and complex mental illness to remain at home for their care whilst
utilising the strengths within the young person, family, or care setting alongside
the multi-agency teams and the community to enable the development of
resilience and recovery, and avoid hospital admission where possible. We also
have a role in supporting hospital discharge to enable shorter stays and
reconnecting with home and community with the additional intervention that may
be required.
We
are looking for a B7 Specialist Mental Health Practitioner to join our highly skilled,
innovative team who share our strengths-based mindset to contribute to our on-going
development in service to do our best for the young people of Devon during this
acute phase of their mental health needs, whilst also improving their personnel
outcomes.
Main duties of the job
We provide support to children
and young people under the age of 18, who are experiencing an acute crisis in
their mental health. This will include completing comprehensive assessments of
mental state and assessing risk. The
successful applicant will act as the lead clinician in the management of young
people with complex mental health needs who require urgent care. These young people will present with episodes of mental ill health and will
often be at risk of admission to CAMHS Inpatient Units.

- Undertake risk assessments and specialist assessments of children and young people with a range of mental health concerns, including but not limited to problematic eating and self-harm
- Work closely with families in order to implement care plans that allow safe management of young people at home, who are often at high risk of harm to self.
- Offer emotional and behavioural support and informed advice and risk management to CYP and their family/carers based on shared knowledge and experience of working with CYP and family/carers with complex needs and at risk of further deterioration of their mental health.
- Ensure all CYP have a clear personalised care plan to promote their development, well-being and protect them from harm. Leading on coordination of support at pace to avoid admission to hospital.
- Work in collaboration with locality CAMHS teams/other agencies to help case manage children and young people highlighted as increased risk of hospital admission, and/or requiring additional intensive wrap around support.
- Provide evidence-based care to young people in crisis and at risk of admission to a CAMHS inpatient Unit/Acute Paediatric Units.
- Liaise with Tier 4 Units regarding admissions and discharge planning.
- Advocate on behalf of CYP and their family/carers where required to ensure the necessary support is in place to enable them to remain within or discharged into the community and promote joined up, consistent care.
- Work within the managing risk pathway Pathway, liaising regularly with the crisis home treatment teams, locality CAMHS teams, Emergency Departments and Paediatric Wards, to aid risk assessment, care planning and implementing care to young people experiencing acute distress in a joined up way.
- Work with existing health, social care, education and other agencies to coordinate packages of care for CYP and their family/carers with complex needs, either at risk of admission to hospital, or currently within hospital settings.
- Hold a County wide position,working across pathways and geographical areas as required, according to need and demand. The role will be aligned closely with a locality team either in Torbay and south Devon, Eastern or North Devon in an attempt to reduce travel.
- Be flexible to the needs of the YP and families/carers, visiting young people at home, school, in community settings and on occasion in the general health hospitals/ in county psychiatric hospitals.
- Provide leadership and supervision to junior staff within the team
- Work flexibly within 37.5 hours per week, including weekends, bank holidays and evenings if required.

We are looking for practitioners who can bring skills, experience and enthusiasm to meet the needs of children/young people with complex social, emotional and mental health needs. The service is currently based in Tiverton but covers countywide, this may subject to change in line with service developments.
