Job summary
The
Homeless Core Mental Health Team works with people in Exeter who have a mental
illness, are experiencing homelessness [rough sleeping, living in hostels and
temporary accommodation or sofa-surfing], and are also suffering the effects of
one or more of the following domains of deep social exclusion:
i) Prolonged institutional care.
ii) Prolonged substance misuse.
iii) Participation in street culture
activities including begging, sex work and survival shop lifting etc.
The
purpose of the team is to provide skilled engagement through relational
expertise, assessment/formulation and case management.
Part
of the role also includes design and co-ordination of treatment packages that
are personalised and evaluated in line with payment by results and providing
statutory Adult Social Care, Care Management functions. The liaison with a wide
range of statutory and non-statutory agencies is central to this role.
The
post will work closely with the Navigator Complex Needs Team based at CoLab,
Exeter. This team provides a multi-disciplinary response (mental health,
substance misuse and housing case work), to a small cohort of severe and
multiply disadvantaged street attached adults with complex presentations and
typically disengaged from support services. The team aims to provide a wrap
around, person-centred and trauma informed response to this client group and
thus seeks to overcome the restrictions of organizational/disciplinary
boundaries.
Main duties of the job
Relationship formation key aspects
of the role are: to establish connection and communication with the
client. Promotion of trust and respect
for client autonomy and being attuned to movement through the stages of
developing a therapeutic relationship. The post-holder will aim to help the
client clarify personally meaningful goals.
A focus on common language construction the
post-holder will work to listen, understand and make use of the clients words, meanings, ideas,
values etc. helping them develop the language to describe their experiences.
Support a process of transition and
adaptation - the post-holder will help the client build
relationships with and adapt to new ideas, people, housing, resources,
services, recovery etc.
Demonstrate the
ability to communicate in a way that is empathic and reassuring whilst
employing negotiating techniques to gain co-operation from highly complex
individuals.
Communicate
information in a way that makes it relevant and understandable for service
users and carers, working in line with practice standards and operational
policies.
Facilitate fair
access to social care services and promote social inclusion regardless of
age, ethnicity, gender, sexual orientation or disability

Qualifications
Essential
- A professional mental health qualification e.g. RMN, Social Work Degree or equivalent, BSc/Diploma in OT, SROT, Chartered Clinical Psychologist, ENB 811/812 or CPN Degree.
Desirable
- Post graduate qualification relevant to specialism.
- Clinical Supervision qualification.
- Training to level 3 in Safeguarding children and young people: roles and competencies for Health Care staff.
Experience
Essential
- Substantial experience of managing an extensive and complex caseload of clients with a range of complex mental health problems.
- Working within a community mental health setting in either the statutory or voluntary sector.
- Providing clinical supervision.
Desirable
- Experience of working in a multi-disciplinary and multi-agency environment.
- Participation in user involvement in services.
