Job summary
We are looking for skilled and highly motivated Assessment and Brief Intervention Workers to join the Exeter Outer Core Mental Health Team. This role acts as the interface between primary care and secondary care mental health services and will be based within GP practices within a specified geographical area.
The service is embarking on a challenging but exciting journey of change with the implementation of the Community Mental Health Framework. The successful applicant will be innovative and compassionate clinician who is motivated to deliver individualised, person-centred care and make a positive contribution to service development. You will join a rich multi-disciplinary team which includes Nurses, Occupational Therapists, Social Workers, Psychologists, Psychiatrists, Peer Support Workers, Clinical Associate Psychologists and Community Support Workers. We are also forming closer working relationships with colleagues in Primary Care and the Voluntary Sector.

Main duties of the job
You will be an experienced mental health professional (Registered Nurse, Social Worker or Occupational Therapist) who has a genuine interest in collaborative working with people of varying ages and complex needs, including service users, their carers and families.
Working as dedicated practitioner to a primary care network, you will triage referrals, complete assessments, signpost to a wide range of community support services and deliver short term, focused interventions to people with a wide range of needs. You will ensure that service users' recovery goals are central to their care and promote social inclusion, independence and wellbeing.
You will work closely with other PCN-based roles to help address the potential range of biopsychosocial needs of patients with mental health problems. This will include the PCNs MAT, including, for example, PCN clinical pharmacists for medication reviews, and social prescribing link workers for access to community-based support.
Operate without the need for formal referral from GPs, including accepting some direct bookings where appropriate, subject to agreement on volumes and the mechanism of access between the PCN and mental health team.
