Job summary
This post in an exciting opportunity for a newly qualified nurse or a nurse who wishes to expand knowledge and experience within Learning Disability Services.
The successful applicant will have the opportunity to work within three Learning Disability Teams that form past of the wider Learning Disability service.
The successful applicant will have access to a wide variety of support that is aimed at supporting the successful applicant in enhancing their learning, skills and development.
At the end of the 3 year rotational cycle the applicant will be offered a role in one of the teams.
Main duties of the job
Details of the three teams included in this post are as follows:
Intensive Assessment and Treatment Team (12 month placement) As part of this team the post holder would be expected to carry out a range of duties in order to support the provision of an effective assessment and treatment response to people with a learning disability who are showing high levels of distress and / or presenting risks to other people.
Continuing Health Care Team (12 month placement) As part of this team the post holder will be expected to undertake specialist nursing assessments for individuals with a learning disability who are eligible for Continuing healthcare. The post holder will ensure the process is implemented in a timely, efficient and transparent manner in accordance with the National Framework. The post holder will be expected to provide case management for people on their case-load that will include planning, commissioning and reviewing packages of care for individuals with complex health needs.
Liaison Team (12 month placement) As part of this team the post holder would ensure that the health needs of the person with a Learning Disability are met when they are referred to the service. This involves communicating with and working in partnership with a range of mainstream clinicians and services working to change systems and educating where appropriate. It also involves close communication and involvement with family, carers and others during the period of care.
