Job summary

We are looking for a Team Manager to lead within our expanding Reconnect Service. The base is The Rowans, Whipton Hospital Exeter, but you will manage teams which cover both HMP Channings Wood and Exeter.

The Service aim is to offer support to individuals as they are released from prison. To ensure that any health vulnerabilities are considered and then given the support to engage with appropriate pathways on release. Helping them integrate back into the community.

Practitioners work with support time recovery workers to engage with individuals prior to release from prison. A Support plan can then be formulated to offer contact on release from prison and on their onward journey in integrating back into the community. Contact will vary in frequency according to need and the peer support workers will engage with individuals to organise and attend appointments with support services to reduce the identified vulnerabilities.

Liaising with partnership agencies including probation and health will be key to trying to establish support and stability within the community. Peer support workers are key to helping and supporting a person on that journey. We work with people with all vulnerabilities including substance misuse, homelessness, social issues, learning difficulties, and mental health. The aim of the service is to improve health and well-being, reduce vulnerability, and the risk of re-offending.

Main duties of the job

You will be an important member of the team, working to ensure that every patient receives high quality care. You will do this through your supervision of the team members and through positive role modelling.

You will often work under pressure and without direct supervision, so we are looking for someone confident and capable. You will prioritise tasks, and manage your own workload, providing support for your colleagues as and when required.

You will be required to support the induction of new staff members and ensure they are competent and confident in their role.

We will help you to grow in the role, and will support you to look and engage with training and development opportunities within the trust.
