We are looking for a committed Health Care Assistant to join our Adult Community Mental Health Services team in Borehamwood. Healthcare Assistants at HPFT are key members of multi-disciplinary teams, supporting adults with mental health needs and helping them build confidence and independence as part of their recovery journey. The post holder will work closely with Nursing team, including the wider Multi-Disciplinary Team (Psychiatry, Psychology, Allied Health Professionals and Arts Therapies etc), to support and implement care plans that promote service user empowerment. Working as part of the team, they will help deliver high quality, individualised care that reflects each service user's needs and supports their overall health and wellbeing.

Main duties of the job

Job Summary

  • Assist with nursing and clinic activities as required/trained.
  • Perform phlebotomy (blood taking) once trained and deemed competent.
  • Develop and maintain knowledge of clinic operations, including monitoring and supporting clinics such as Physical Health Check, Clozapine and LIA clinics.
  • Liaise withDMS and Pharmacy, including ordering medication and providing support within the clinic environment.
  • Liaise with consultants and medical staff regarding prescription charts, ensuring accurate communication and documentation.
  • Implement agreed care plan and therapeutic interventions under the guidance of registered practitioners supported by on-going supervision and support.
  • Support the wider multi-disciplinary team, including health and social care professionals.
  • Contribute towards the ongoing quality of service and care and identify any risk issues in within an agreed and monitored supervisory framework.
  • Accurately monitor and provide timely feedback on agreed care strategies and interventions to registered practitioners to support the evaluation of their success.
  • To work within the Care Programme Approach / Care Management process with a focus on Recovery Principles
  • To provide support, give time and thus promote recovery
