An exciting opportunity has arisen for a Mental Health Nurse Band 6 to work within the HPFT Integrated Care Team within the Hertfordshire area. Our Integrated Care Mental Health team (ICT) are a team of professionals who specialize in working with people who are have long term physical health conditions, including frailty, which may be impacting on their mental health. Our team aims to support service users to continue to live independently in their own home.
This is a unique position for someone who enjoys teamwork, challenges and variety in their day to day work. You will be working collaboratively with other health colleagues with service users who have complex frailty needs that are impacting on their mental wellbeing. This position is based in the Hertfordshire area, supporting our service users with mental health and wellbeing. In their own home, this will include nursing and residential homes.
Main duties of the job
You will be expected to:
- Visit service users who are identified under the frailty pathway, in their place of residence, to complete a holistic assessment of their physical and mental health needs.
- Required to take responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care and management of a defined caseload, including liaison with other agencies and, where appropriate, the supervision, development and teaching of staff and/or students.
- Work in partnership with other healthcare professionals and trusts. Integrated Care Teams aim to make healthcare more preventative, this service supports service users with multiple long term or complex conditions to remain at home rather than going into hospital or residential care. Referrals are accepted from various sources such as primary care, ambulance service, A&E and secondary care.
- Hold a case load of service users, ensuring the care plan is met at all times.
- Be part of an wider supportive and holistic multidisciplinary team of mental health nurses, mental health OT's and Psychology team.

- Degree, diploma or equivalent in registration Nursing (Mental Health Nursing)
- Registration with Nursing and Midwifery Council
- Mentorship Preparation Course.
- Two years post registration experience, with at least one year post registration experience in Older Adult Mental and or Frailty health care
