A dynamic new role has been created for a Community Mental Health nurse to work in the FACT service (Flexible Assertive Community Treatment)!
We are seeking to recruit an experienced mental health nurse who is keen to work with service users who require immediate support to prevent escalation and admission to hospital.

You must hold a full valid driving license and have access to a car to use regularly for business purposes (unless you have a disability as defined by the Equality Act 2010).
This role attracts a recruitment incentive of £6,000: £1,500 on commencement, £1,500 after six months and £3,000 every six months thereafter
Main duties of the job
The successful candidate will:
- be responsible for the clinical and operational day to day service delivery of care to service users who are receiving support from FACT
- undertake assessments of needs with recommendations around care planning and risk management
- undertake Connected Lives assessments including physical health assessments and assessments of medication side-effects
- participate in Multi Disciplinary Team working
- supervise Band 5 Nurses, Nursing Associates, Health Care Support Workers and Support Time and Recovery Workers and students.
