We are looking for Band 6 Occupational Therapist to join the exciting development of an Integrated Community Forensic Learning Disability Team based at Little Plumstead Hospital, Norwich. This team is part of an initiative from the recently formed NHS East of England Provider Collaborative's plans for accelerated transformation in forensic learning disability services. Its focus is on improving continuity of care for patients. The expectation is that this approach will not only lead to reductions in length of hospitalization for patients within forensic learning disability units, but also minimize the need for in-patient admissions in the first place. The Community Team will build upon the success of our existing Outstanding CQC rated forensic services.

Main duties of the job
- As a band 6 Occupational Therapist you will hold a caseload of patients who have a learning disability and may have co-morbid mental health issues.
- To undertake specialist risk assessment such at HCR20v3, ARMIDILO-S (training provided)
- Work as part of the MDT, and represent OT within team meetings.
- Attend professionals meetings.
- Work with the patients to use a range of MOHO based assessments and goal setting.
- Work with the patients from initial assessment, throughout their treatment pathway and on to discharge.
- Deliver professional and team based training within the trust and to partner organisations.
- Supervise band 5 OTs and OTAs.
- Offer student placements.
- Raise awareness of Occupational Therapy and offer staff training as appropriate.

About us
The Community Forensic Learning Disability Team will be part of Hertfordshire Partnership University NHS Foundation Trust which is at the forefront of academic and research activities in the field of developmental disabilities and hostsRADiANT(Research in Developmental Neuropsychiatry), a network of NHS Trusts, university academics and experts by experience. The post holder will have ample opportunities to benefit from these academic links.
A key feature of the team will be its close working relationship with the secure inpatient learning disability service, Broadland Clinic, a 25-bed medium secure unit for patients with developmental disabilities who are detained under the Mental Health Act. This will include its team members being embedded into the in-patient teams and vice versa. The firm expectation is that staff in both services will work in an integrated manner to promote treatment continuity for patients and ensure better treatment outcomes.

- To participate in the planning, development and evaluation of OT services, holding responsibility for defined projects where appropriate
- To regularly supervise OT students on practice placement, following attendance at an approved Fieldwork Educators course
- To apply knowledge of the Model of Human Occupation (MOHO) and other standardised assessment approaches relevant to the clinical setting.
- Following assessment, to undertake case formulation in order to highlight occupational strengths and limitations and to inform the intervention plan moving forward.
- To work collaboratively with service users to identify OT goals as part of the overall recovery plan.
- To provide interventions in the manner that is consistent with Trust standards and values.
- To attend relevant MDT and professionals meetings for you case load.
- Provide written reports as required.
- To routinely provide feedback to the MDT regarding observed occupational performance throughout contact, which inform ongoing assessment and therapeutic intervention.
- To provide specialist support and education to service users and carers regarding aspects of occupational therapy e.g. equipment fitting, rehabilitation programmes.
