Job summary

Essex Learning Disability Partnership are looking for a Registered Health Professional with a passion for delivering the highest standard of care for individuals that have a learning disability.

You will be working as a Community Nurse Practitioner within the Mid West Essex Community Learning Disability Team, based at Technicon House in Braintree.

Our Community Learning Disability Teams provide person centred and holistic assessment and care for adults with a learning disability living in the community. The multidisciplinary team works with individuals that live independently in the community, with family/carers, and those that reside in supported living schemes or 24-hour residential placements. The service is committed to improving the health and well-being of people with a learning disability across Mid West Essex, by working in a simple, person-centred way that empowers individuals to maximise their potential.

The successful candidate will receive up to £120 towards their registration fee, which will be paid upon commencement. This payment will be subject to tax and national insurance deductions.

Main duties of the job

  • Assessing the health care needs of people with Learning Disabilities in a range of environments and implementing interventions as required.
  • Working in an interdisciplinary way to assess service user's needs, devising care plans and evaluating outcomes.
  • Working directly with people who use our services, their carers, colleagues and other service providers.
  • Assisting and being proactive in health promotion for people with Learning Disabilities, ensuring inclusion into mainstream services in line with national and local policy and guidance.
  • Delivering training and education to other professionals/carers/family.

  • Experience of communicating calmly, efficiently and in a friendly manner, using a variety of methods, with people who require support, members of the MDT, carers/family and other agencies involved in the persons care.
  • Experience of maintaining high standards of record keeping.
  • A good understanding of legislation and national guidance relating to the care and support of people with a learning disability.
