We are excited to implement the role of Senior Primary Care Mental Health Practitioner (Band 7) in West Hertfordshire's North Watford Primary Care Network (PCN) covering the following GP surgeries in the North Watford and Abbots Langley area:

Sheepcot Medical Centre, 6 Cunningham Way, Leavesden, Watford WD25 7NL

Our PCN is the smallest in the CCG area and all 3 GP practices are within a mile radius of each other - this makes it easy to travel between sites. The practices all serve the same village. From a clinical point of view, we are like-minded with strong relationships. The two larger practices are training practices so we are used to, and like having new team members joining us all the time.
Main duties of the job
The successful candidate will provide mental health assessment and support/brief interventions to service users in GP surgeries. This is an opportunity to work innovatively and flexibly to make it easier for service users to access the help that they need.
Your tasks will include early triage, assessment, treatment, risk management, identifying and dealing with safeguarding concerns as well as contributing to case discussions and service-related meetings across the adult mental health primary care pathway ensuring good links with secondary care services.
You will coordinate the time limited care of primary care-based service users delivering integrated care which takes account of social care needs. You will be someone who enjoys adapting and learning as you contribute to the embedding of this new role in primary care.
Nurse applicants will fulfil the role of a Nurse Independent Prescriber as per Primary Care policy, Trust policy and NMC requirements - or be willing to undertake training for this and will be supported into doing this as part of their development.
You will also take a leading role for service innovation and development in primary care mental health. This could include projects with other junior staff to ensure delivery of safe and effective primary care mental health services and making a significant contribution to the development and leadership of mental health services within Primary Care Networks.
