Job summary

We are looking for full time, Band 4 FACT (Flexible Assertive Community Treatment) Support Worker to join Adult Community Mental Health Team based in Colne House, Watford (The role will also cover Borehamwood and Potters Bar). You will provide intensive support to service users within South-West, Hertfordshire. You will need experience of working with people who have mental health issues in the community. Your role as support worker with FACT will be to work as part of a mini team within the wider multi-disciplinary team, and support the community treatment of people with mental health needs and their families.

Main duties of the job

  • You will work within a Multi- Disciplinary Team and will carry out direct Service User work under the supervision of a qualified practitioner.
  • You will participate in assessing the specialist needs of people with a mental health illness living in the community, or as inpatients in Hertfordshire Inpatient services and in the implementation of their individual care plans.
  • You will develop and maintain professional working relationships with service users, carers, team colleagues and other agencies.
  • You will work flexibly over a 7 day period in a variety of health and social care settings.
  • You will work as a lone worker using delegated autonomy and initiative.

QUALIFICATIONS/EDUCATION/TRAINING

Essential

  • Evidence of academic achievement to at least GCSE level or equivalent in English, Maths and a Science subject
  • NVQ level 3 qualification or mental health certificate level 3 or agreement to undertake such training as soon as resources permit
  • NVQ/Mental Health Certificate Level 2
  • Basic food hygiene
  • Basic first aid skills
  • Basic health and safety awareness
  • SCIP Trained

Desirable

  • Venepuncture training

PREVIOUS EXPERIENCE

Essential

  • Experience of working with people who have learning disability and who may have mental health needs and/or challenging behaviour
  • Relating to People Skills
  • Moving and handling awareness
  • Awareness of local services
  • Knowledge of benefits/employment systems
