Job summary
An exciting opportunity has arisen for a Floating Mental Health Nurse to work within the HPFT Integrated Care Team across East & North Hertfordshire. This is a unique position for someone who enjoys teamwork, challenges and variety in their day to day work.
Main duties of the job
You will be working across different localities in East and North Hertfordshire supporting the locality community mental health nurses, mental health OT's and Psychology team. You will be working in primary care alongside community matrons, occupational therapists, physiotherapist, social workers, and other professionals.

You will visit service users who are identified under the frailty pathway, in their place of residence, to complete a holistic assessment of their physical and mental health needs. You will work in partnership with other healthcare professionals and trusts. Integrated Care Teams aim to make healthcare more preventative, this service supports service users with multiple long term or complex conditions to remain at home rather than going into hospital or residential care. Referrals are accepted from primary care, ambulance service, A&E and secondary care.
