Job summary

We have an exciting opportunity for a Mental Health Nurse to bring their expertise into the teams that support people following discharge from the district general hospital into care home for further assessment, support and rehabilitation.

  • This really is an exciting opportunity to work in an integrated way, not only across the organisations that provide Physical and Mental Health as well as with Social Care; but also across a number of sites - for example district general hospital, Nursing and Residential Homes, community health buildings and in people's own homes.We can be flexible with the base, for example Hemel One, Apsley One or Watford General Hospital.

Main duties of the job

You will be part of the integrated care provided by HPFT in supporting people with complex physical health/frailty as well as mental health needs. You will primarily work within the Discharge to Assess and Post Hospital Review Teams at Watford General Hospital to:

  • Provide mental health assessment, and risk assessment (to people in Discharge to Assess beds, or open to the Post Hospital Review Team)
  • Provide support, advice and guidance to the Discharge to Assess and Rapid Response Teams, and to the Post Hospital Review Team.
  • Support the Discharge to Assess Single Point of AccessSupport ward staff at the district general hospital with risk assessment, risk management, and positive risk taking where people are under 1-1 observations.
  • Review people for MHSOP who have previously been seen by Mental Health Liaison, who are now in a Discharge to Assess bed.
  • As part of MHSOP you will have access to the supervision, support and training that we offer, but as part of the Discharge to Assess and Post Hospital Review Teams, you will also develop your knowledge and skills in physical health and social care.

Essential

  • Degree, diploma or equivelent in Nursing (Mental Health Nursing
  • Registration with Nursing and Midwifery Council
  • Mentorship Preparation Course.
  • Two years post registration experience, with at least one year post registration experience in Adult Mental Health Care.
  • Good knowledge of Microsoft Word, PowerPoint and Excel
  • Knowledge of legislation related to mental health and social care (e.g. Mental Health Act, Mental Capacity Act, Care Act, Care Programme Approach etc)
  • Ability to manage distressing and challenging situations

Desirable
