Registered Mental Health Nurse: Community Rehabilitation
The Enhanced Rehabilitation Outreach Service (EROS), which is part of HPFTs wider Rehabilitation Services is looking for an enthusiastic and proactive Nurse to join our award winning and well-established team as it takes part in an exciting service expansion.
The EROS team is instrumental in supporting service users with serious mental illness. They support service users to be independent through providing recovery focused interventions within Community Rehabilitation pathways. The service supports individuals to co-produce plans of care. They work with service users to build resilience and improve their wellbeing, tailoring interventions to meet their individual cultural and social needs.
The EROS team is a county-wide and therefore car driving and ability to drive distances across the county is essential unless you have a disability (as defined by the Equality Act 2010) which prevents you from driving.
The service operates across 7 days on a rota basis. Overnight working is not required.
Main duties of the job
The successful candidate will be a professional who is passionate about empowering people to live independently, engage with their local community, and promotes a positive attitude with every individual using the service. They will play a key part in developing the service.
Key responsibilities will include:
- Providing nursing assessment and treatment to service users
- Providing high quality, individualised interventions
- Managing a caseload of service users with complex Mental Health needs
- Contributing to the development and implementation of the Enhanced Rehabilitation Outreach Service
- Providing leadership to junior staff, through supervision and appraisal
- Working with a range of professionals, service users and carers
- Implementing and promoting evidence based clinical processes and standards of practice
- Undertaking a range of comprehensive and evidence based clinical risk assessments and associated care plans that are relevant to the service user's care needs
