Job summary

An exciting opportunity has arisen within in Essex to be part of a new Physical Health Team providing services to people with Severe Mental Illness. HPFT are looking for people who want to make a difference to their community, supporting patients with mental health difficulties, and working alongside GPs and other professionals.

The successful candidate will work alongside primary care under the clinical supervision and oversight of the Senior Service Line lead and Head of Nursing.

Main duties of the job

The primary responsibility for this post is to provide a comprehensive and systematic annual health to those on the Severe and Enduring Mental illness register (SMI) and provide leadership and direction for the physical health team, developing working partnerships with primary care and wider system, in the context of contemporary service strategy and operational objectives.

These new opportunities within primary care will be developing and expanding over the next three years, and will be a core feature of delivering excellent care to people in the local community.

Work as part of the Primary Care Team to minimise duplication andcoordinate health checks Ensure appropriate physical health observations and blood tests aretaken and actioned for all patients who are eligible for an annualphysical health check. This will involve liaison with GPs, practicenurses, practice managers and other primary care staff Liaise with Primary Care professionals to ensure any follow up actionsare completed as part of Annual health check process. Offering health and lifestyle advice, signposting to relevant informationand services, delivering interventions or liaising with GP to do so. Provide clinical leadership in their service area. This means keepingclinically up to date, being able to assess patients thoroughly,plan care rigorously, and audit and evaluate nursing interventionsbased on evidence and locally agreed protocols and policies. Will be responsible for communicating any identified mental health,physical health, social care and safeguarding issues to relevant staff. Provide effective professional support to the physical health team. Provide practice supervision and support for nursing students andstudent nursing associates on placement in the service. Provide nursing care in a person centred way that promotes andfacilitates the positive health and well-being of people experiencingmental health problems and learning disabilities. Will maintain close liaison with all department\staff involved in patientscare and report on progress and effectiveness to the MDT. Will be aware of all policy documents and legal requirements andGeneral practice local procedures pertinent to the post and ensure thatthese are adhered to. This must link to SystmOne Arden procedureswhich is linked to NICE Will demonstrate and serve as an example for all clinical activities toappropriate grades of staff.   • RMN Managing team
  • Current registration with NMC
  • ENB 998 / Preceptorship and Practice Supervisor and Assessor Training.
  • At least 3 years post registration experience at Band 6.
  • Community nursing experience.
  • Venepuncture competent.
  • Interpretation of blood test results.
  • Ability to identify and intervene for health and lifestyle factors.
  • Evidence of continuing professional development.

Desirable
