Job summary
We are seeking highly motivated individuals to join our established Hospital Palliative care team in Worcestershire Acute Hospitals NHS Trust, who provide a 7 day specialist nursing service.
As part of the ICB End of Life care strategy we are working in partnership towards Implementation of the Gold Standards Framework across the ICS. This an exciting opportunity to practice within a combined clinical/facilitator role based at Worcester Royal Hospital.
The applicants should have substantial post registration experience in palliative care nursing, and be educated to at least first degree level. You should also hold a teaching qualification and have completed Advanced Communication Skills Training. The ability to work in partnership with all members of the multi-disciplinary team to provide seamless care and support is also essential.

Main duties of the job
To work as a member of the Specialist palliative & End of Life care team, as part of the wider primary and secondary muti-disciplinary teams and the independent sector to ensure a cohesive, holistic and research-based approach to the management of palliative care patients.
Collaborating with colleagues across the ICS to actively support and facilitate the ongoing promotion and implementation of the Gold Standards Framework model within Worcestershire Acute Hospital
Working with colleagues in the Specialist palliative & EOL care team, to facilitate the on-going promotion and implementation of the Individualised Last days of Life care plan in the acute hospital setting alongside evaluation of outcomes.
To work collaboratively with acute hospital teams to uphold the principles of choice, comfort, and dignity for individuals approaching the end of life. This includes promoting best practices in advance care planning, facilitating compassionate and informative conversations, supporting medication review and rationalisation, ensuring clear documentation, and enabling discharge to the individual's preferred place of dying where appropriate.
To promote and participate in joint working with acute hospital colleagues, to encourage and support them by sharing expertise, good practice, experience and promoting a culture of learning.
