Job summary

Part-time working pattern - 4 days per week, 7.5 hours per day.

This is an exciting and ever-changing role for somebody who is passionate and enthusiastic about lung cancer care. There is a great opportunity to work in partnership with members of the Lung Cancer Multidisciplinary Team to provide a seamless service for patients and carers.

Main duties of the job

Under the guidance of the Band 7 Lung Cancer Specialist Nurses the post-holder will have the opportunity, through work-based learning and training, to develop specialist knowledge and skills enabling autonomous practice within the speciality of lung cancer.

To manage patients from pre-diagnosis, diagnosis and through to advanced disease management.

Contact with the patient and carers will be through a variety of methods; face to face, telephone, and video conferencing to assess, develop, implement, and evaluate an individualised plan of care.

The Lung Cancer Specialist Nursing team take every opportunity to provide a range of educational and training packages for health care staff and patients to disseminate quality care.

There will be opportunities to work alongside the Band 7 CNSs and wider MDT members in development of the lung cancer pathway with a view to continually improving patient experience and clinical outcomes.

There will be opportunities to actively undertake and participate in clinical audit, research, and service evaluation within this speciality.

Key Duties:

1. Provision of Specialist Clinical Services

The post-holder will:

1.1 Act as a clinical role model and demonstrate specialist knowledge of lung cancer and mesothelioma in the acute hospital setting under the expert guidance of the Band 7 Lung Cancer Specialist Nurses.

1.2 Make and receive referrals from other health care professionals under the guidance of the Band 7 Lung Cancer Specialist Nurses.

1.3 To assess, develop, implement, and evaluate an individualised programme of care for patients with lung cancer and mesothelioma.

1.4 Provide patient and carer education and refer to other health care professionals when appropriate by using a variety of written and verbal information, for example, support groups, pre-habitation/pulmonary rehabilitation courses, smoking cessation etc.

1.5 To be responsible for the on-going management of a caseload of patients with lung cancer or mesothelioma. To organise the caseload effectively and efficiently regarding clinical priorities and time management.

1.6 Under the guidance of the Lung Cancer Multidisciplinary team order appropriate investigative tests.

1.7 Be present at the time of diagnosis of cancer to provide specialist nursing support to encompass physical, psychological, social, and spiritual needs of the patient, family, and carers.

1.8 To communicate effectively with patients and carers to maximise rehabilitation potential and ensure understanding of condition to enable effective self-management.

1.9 To assess capacity and have the ability to work within a legal framework with patients who lack the capacity to consent to treatment.

1.10 Communicate complex patient related information effectively with MDT members across health and social services, private providers, and other agencies; to ensure the delivery of a co-ordinated multidisciplinary service.

1.11 Participate in the Lung Cancer MDT for the management of patients ensuring a best practice approach.

1.12 Maintain accurate record keeping which contribute to the national statistics through completion of Summerset Cancer Registry.

2. Education and Training

2.1 Participate in the Trust personal development review (PDR).

2.2 Provide specialist advice, teaching, and training of other healthcare professionals with regard to patients with lung cancer and mesothelioma on a formal and informal basis.

2.3 Commit to a philosophy of care which has an emphasis on self-management and patient empowerment through undertaking health education and health promotion activities.

2.4 To actively participate in audit projects and patient surveys where appropriate, report findings to the multidisciplinary team to further develop an ever-changing service to improve the patient experience.

2.5 Be part of an expert team who want to share specialist knowledge and skills to both pre and post registration nurses, doctors, and allied healthcare professionals.

3. Professional

3.1 Be professionally and legally accountable for all aspects of own work; including caseload management. Ensure a high standard of clinical care is provided to your patients by undertaking further training and academic qualifications as relevant to the role and service requirements.

3.2 Demonstrate a working knowledge of clinical governance and risk management strategies relevant to the role.

3.3 Be aware of Health and Safety relevant to the role and comply with any policies which may be required to maintain and improve the safety of your work area, including the prompt recording and reporting of incidents or accidents via the appropriate channel e.g., Datix.

3.4 Maintain own continuing professional development records and assume responsibility for remaining fit to practice within your profession.

3.5 Participate and be the patients advocate in the Lung Cancer Multidisciplinary Team meetings to ensure patients wishes are expressed and feedback discussions to the patient, family, and carers.

3.6 Be aware of research/ trials being undertaken within the speciality of lung cancer and mesothelioma.

3.7 Promote team and collaborative working practices by effective communication and liaison with all members of the multidisciplinary team.

3.8 Develop links and collaborative working relationships which enhance the interface between staff, patients, community, and visitors across organisational and professional boundaries.

3.9 Participate in and contribute to innovation, service redesign or review where required, ensuring these are linked to organisational priorities and support improved patient clinical and experience outcomes.

3.10 Provide and accept constructive feedback and use this to reflect on your own practice by attending clinical supervision as offered.

4. Risk Assessment:

4.1 This post involves regular close contact with patients, potentially exposing the post holder to infections and bodily fluids e.g., sputum, urine, blood, faeces, and vomit.

4.2 The post holder will be routinely involved in the counselling and management of patients and carers under difficult situations, e.g., around a newly diagnosis of cancer, the treatment options and palliative care.

4.3 The post holder may be asked to carry a bleep and be available to hospital staff, having to prioritising incoming referrals.

4.4 The post holder will use a computer VDU for up to 50% of a working day.

4.5 The post holder will be required to drive between hospital sites.

4.6 The post holder will be required to work in a variety of care settings within the hospital e.g., Wards, A&E, MAU, outpatients (respiratory/oncology/surgical), endoscopy etc.
