We manage acute haematological malignancies for patients throughout
Herefordshire and Worcestershire with this role being an integral part of the
MDT along with consultants and specialist nurses.
We
are looking for an enthusiastic pharmacist to join our cancer
services team in this key leadership role to work alongside our highly
experienced cancer services team.
You will be responsible for managing haematology patients on our dedicated
haematology ward alongside mentoring and coaching of band 6 and 7 pharmacists.
You will also be able to maintain skills within our aseptic unit.
As a department, we are committed to the development of pharmacists into

Main duties of the job
Be responsible for
leading and delivering the highly specialist clinical pharmacy services to Haematology
patients within the trust.
Function as an independent prescriber.
Lead, develop,
evaluate and deliver pharmaceutical care to patients managed by the Haematology
teams within the trust.
Use specialist knowledge to advise consultants, medical, nursing
and pharmacy staff on all pharmaceutical aspects of care.
Recommend,
substantiate and communicate best therapeutic options for patients.
Lead and develop
clinical audit of medicine use and pharmacy services within haematology.
Introduce measures
to keep the directorate drug expenditure within agreed budgets.
Deliver pharmaceutical teaching to
medical and nursing staff within the Haematology teams and wards.
Deliver structured
training programmes for Foundation Year Pharmacists and junior pharmacists as assigned; contribute to the development of training and education programmes
for pharmacy staff
Ensure that good manufacturing
practice, medicines legislation and agreed protocol driven treatment regimens
are adhered to on a day to day basis, resulting in highly risk managed services
for staff and patients.
Work closely with
the Clinical Team Lead/ACP lead for Cancer services in the scoping
of an advanced practice role in the management of adult Haematology patients

Job responsibilities
Lead and deliver a highly specialist pharmacist role to adult
Haematology team patients at WRH.
Function as an Independent Prescriber within an allocated
clinical area or to a defined cohort of patients. This will be in line with GPhc registration
requirements and be formally agreed with all appropriate and relevant
Clinicians, as well as being within the departmental scope and funding for IP
practice.
As an IP pharmacist the post
holder will, practice within the scope of GPhC registration and own competence,
adhere to the Medicines Policy and the Trust NMP policy, in relation to scope
and responsibilities of role, comply with the Trust Medicines formulary, be
responsible for providing on-going evidence of competency in relation to their
prescribing role
Provide a highly specialist clinical pharmacy service to
patients and undertake pharmaceutical risk management for any allocated
wards/clinics.
Ensure compliance with medicines legislation, the Trust
Medicines Policy, and medicines related national and local guidelines within
the Haematology directorate.
Produce appropriate technical and professional SOPs and
associated policies to ensure that the Haematology directorates medicines
management systems are safe, robust, guided by national directives, standards,
guidelines, and the post-holders own specialist knowledge, and are quality
assured
Evaluate prescribing practices and amend according to
evidence-based practice, national/regional directives and clinical governance
targets daily, during consultant ward-rounds, and at the weekly multidisciplinary
team meeting.
Construct specialist pharmaceutical care plans for patients,
assess and monitor clinical progress and outcome regarding their medication and
actively intervene to ensure that patients receive optimal care with minimal
side effects.
Provide expert advice to health care professionals on all
pharmaceutical issues arising within the post holders specialist clinical
area. This will include the extrapolation of clinical and complex medicines
information and clinical trials where data may be lacking and analysing,
interpreting and comparing differing opinion or facts to determine the best
prescribing strategy for an individual patient or patient group.
Be responsible for the therapeutic blood monitoring of
identified drugs within the post-holders clinical area.
Ensure that information and patient counselling systems are
implemented within their specialist area according to patient need to ensure
that patients obtain the most benefit from their medicines.
Set standards and manage the counselling of patients on
their medicines within the Haematology directorate to improve concordance and
knowledge of their therapy.
Ensure all newly admitted patients are interviewed to
construct an accurate medication history and assess compliance/concordance
issues.
Review the medication history against the admission details
and prescription chart.
Pro-actively identify to medical staff any potential or
actual medicines-related hospital admissions or associated issues and outline
any required action to resolve identified discrepancies
Assess patients discharge medicines information
requirements as early as possible in the admission stay to be able to prepare
information on medicines for patients based on individual need.
Counsel patients with highly complex needs such as a
physical disability which may affect information requirements (sight or
hearing), language difficulties, those with life-threatening or terminal
diagnoses and highly distressed patients.
Helping to provide a cost effective and quality assured
aseptic service at WRH that is risk managed and meets the clinical and business
strategies of the Trust.
Undertake pharmaceutical risk management for the aseptic
services provided and ensure medicines legislation is complied with in those
areas.
Providing a specialist aseptic pharmacy service to
Haematology and Oncology patients and ensuring the chemotherapy regimens
prescribed for patients by haematologists and oncologists at WRH are safe and
appropriate for administration to individual patients given their diagnosis and
health status, are in line with standard protocols or approved clinical trials,
and are prepared accurately and safely in a timely manner.
Support in the provision of a specialist aseptic pharmacy
service to Paediatric patients and ensuring the chemotherapy regimens prescribed
for patients by paediatricians at WRH are safe and appropriate for
administration to individual patients given their diagnosis and health status,
are in line with standard protocols or approved clinical trials, and are
prepared accurately and safely in a timely manner.
Ensuring individual patients entered into clinical trials
are treated according to the trial protocol, that their medicines are prepared
as specified in the protocol, that any randomisation codes are allocate
appropriately, and that all records are maintained correctly and stored
suitably.
Substantiating clinical pharmacy recommendations at the
highest level and against challenge from Consultants, where necessary, and
effectively communicate any required actions to other medical and nursing
staff.
Providing specialist clinical advice to other health care
professionals on all pharmaceutical issues arising within the specialist area
of Aseptics. This will include the extrapolation of clinical and complex
medicines information where data may be lacking and analysing, interpreting and
comparing differing opinion or facts to determine the best prescribing strategy
for an individual patient or patient group.
Managing the accurate and timely dispensing and supply of
medicines to haematology, oncology and paediatric patients.
Adherence to NHS England standard operating procedures in
relation to drugs supply through the Cancer Drugs Fund (CDF) and other
commissioning arrangements.
Supervise and train junior pharmacists, Foundation Year
Pharmacists and other pharmacy students in the provision of a safe and risk
managed aseptic service.
Assisting in the production of SOPs to support the use of
the chemotherapy prescribing and administration system.
Providing support and advice for prescribers and nursing
staff when needed in the use of the chemotherapy electronic prescribing and administration
system.
Manage
pharmacists as allocated including delivery of PDR and management of sickness
based around trust policies.
Advise and guide Consultants on proposed new medicines for
use within the post-holders clinical area and assist with compiling new product
assessments and evidence-based reviews for submission to the Area Prescribing
Committee for formulary and funding approval.
Participate in the weekend, Bank Holiday and on call rotas
