Alexandra Hospital, Redditch.
The post offers a skilled post
diploma clinical pharmacist the opportunity to deliver and further develop
pharmacy services to respiratory care and help to shape the development of
pharmacy staff of the future.
You will have experience of
pro-actively influencing prescribing within a secondary care environment, hold
a post graduate clinical diploma and ideally an independent prescribing
qualification and above all, be a team player.

Main duties of the job
- Lead and manage the clinical pharmacy service to respiratory medicine at the Alexandra hospital, Redditch
- Attend and actively participate in board/ward rounds as part of a multi-disciplinary team
- Prescribe and monitor treatment for respiratory inpatients to ensure optimal evidenced based care and expedite timely, safe discharges
- Provide expert advice to other health care professionals on all pharmaceutical issues arising within the specialist clinical area
- Role model best practice and individualised person-centred care and be a source expert of knowledge for individual staff and clinical teams
- Work to develop and expand pharmaceutical services for respiratory patients including exploring options for pharmacist outreach services and pharmacist led clinics
- Support and contribute to the wider specialty medicine team providing leadership
- Proactively work within the directorate to develop guidelines and clinical strategies for trust wide implementation
- Contribute to the training and education of junior colleagues in formal roles such as Designated Supervisor for foundation trainee pharmacists/Designated Prescribing Practitioner for those undertaking Independent Prescribing. Also participating in and developing training and education to the wider pharmacy team

Job responsibilities
The
post host holder will:
Be
responsible for delivering the specialist clinical pharmacy to Respiratory medicine.
Deliver an
embedded medical team pharmacy service, using specialist knowledge
of medicines to provide advice to consultants, medical, nursing and pharmacy
staff on all pharmaceutical aspects of medicines for patients.
Optimise
pharmacist practitioner and prescriber opportunities within Speciality Medicine
and facilitate pharmacist prescribing initiatives.
Deliver
pharmaceutical care to patients within Speciality Medicine at AH; recommend,
substantiate and communicate best therapeutic options for patients and
implement changes in prescribing practice to ensure evidence-based medicine is
followed as appropriate.
To function
as an independent prescriber within Respiratory medicine as agreed with
clinicians and funding, complying with all requirements of the GPhC in relation
to registration as an independent prescriber and the associated professional
responsibilities.
Identify medicine-related causes for admission
and develop treatment plans to optimise care.
Undertake
risk management for the pharmacy service to allocated wards and ensure
compliance with medicines legislation, the Trust Medicines Policy, and
medicines related national and local guidelines.
Undertake
clinical audit of medicine use and pharmacy services within the Medical
Division.
Ensure
pharmaceutical aspects of research and clinical trials in medicine comply with
relevant legislation and conduct own practice-based audit/research on
pharmaceutical aspects of acute medicine.
Provide specialist
pharmaceutical teaching to pharmacy, medical and nursing staff
Introduce
measures to ensure drug expenditure is within agreed limits across the
Alexandra hospital as requested by the Director and Deputy Director of
Pharmacy. This will include monitoring the use and funding of new medicines for
the Area Prescribing Committee (APC)
Function
as an Independent Prescriber ensuring appropriate
medicines are prescribed or de-prescribed after assessment. This will be in
line with GPhC registration requirements and be formally agreed with all
appropriate and relevant clinicians, as well as being within the departmental
scope and funding for IP practice. As an IP pharmacist the post holder will:

Practice
within the scope of GPhC registration

Adhere
to the Medicines Policy in relation to scope and responsibilities of role

Comply

responsible for providing on-going evidence of competency in relation to their
