Job summary
In
collaboration with the Hospital Palliative Care Team, provide expert advice and
skills to facilitate learning and development around the use of Uncertain Recovery/AMBER Care and how it benefits patient outcomes.
The role
will be innovative and encompass all aspects of the patient care pathway, acting
as a nurse advisor to support patients and their carers.
To educate and support ward staff to recognise in a
timely manner when Uncertain Recovery/ AMBER Care should be implemented in order to monitor
and review closely patients whose recovery is uncertain and ACT accordingly.
To support the use of Uncertain Recovery/ AMBER Care as a tool to
elicit preferences and support discussions around aspects of future care.
To facilitate and deliver improved patient flow for
patients with long term conditions and cancer at the end of life stages across
Worcestershire Acute Hospitals NHS Trust when Uncertain Recovery/ AMBER Care is used.
To enable health and social care professionals to
develop communication skills required to support patients to make decisions around
choices when coming towards the end of their life.
As a member of the Palliative Care Team,
support team activity in relation to Uncertain recovery /AMBER Care, where no Specialist
Palliative Care input is needed.

use of Uncertain Recovery/AMBER Care
Main duties of the job
Assist the multidisciplinary team in delivering
and evaluating the interventions/ plan of care, reviewing and amending this
care as appropriate with support from the CNS
Frequently
act as a primary resource for site-specific specialist nurses, nursing/medical
teams and the multidisciplinary team.
Use a
palliative care database that includes patient registration, clinical details
and interventions that are shared with the community palliative care team, the
consultant in Palliative Medicine, the Hospices and the inpatient palliative
care unit.
Ensure staff are aware of prognostic indicator guidance to
help decision- making around issues that those with an Uncertain Recovery/AMBER Care Bundle in
place may have.
Serve as an expert resource for healthcare professionals, including
health and social care professionals in relation to Uncertain Recovery/ AMBER Care and the
use of advance care planning.Effectively utilise informal education opportunities with
patients, carers and professionals.
Research and Audit
Develop and participate in appropriate nursing and medical
research.
Regularly audit issues relevant to current service provision,
reviewing and amending practice as a result.
Be involved in clinical effectiveness issues, particularly the
introduction of patient pathways e.g. working with the End of Life Team.

forms and documents, accurately input data onto and manage a large database and
