Job summary

An Exciting opportunity has arisen for a Cancer Tracker within the Dermatology Service. The aim of the Cancer Tracker role is to help support and navigate patients, their families and carers through a skin cancer pathway.

The service's prime aim is to be patient-focused and proactive in seeing that the patient receives the very best cancer pathway experience with continual communication and support in a timely manner.

You will be involved throughout the patient journey from referral to follow up within the hospital setting, working directly and indirectly with patients and their carers, assisting the clinical team in coordinating the patient pathway. You will be expected to assist with Multidisciplinary Meetings and ensuring documentation is clear, accurate and up to date.

The role requires someone with exceptional organisational skills who can demonstrate great communication and interpersonal skills.

A caring and sensitive attitude is essential as you will be dealing with patients, their families and carers. You will have excellent listening skills and be able to deal with people both internally and externally in a sensitive and professional manner and be confident using patient administration systems and database packages.

The post holder will be expected to work independently and will actively support and contribute to the requirements of the service and ensure compliance with performance targets and quality standards required at a local and national level.

Main duties of the job

The post will focus primarily on working collaboratively with the clinical team supporting the diagnostic pathway to ensure patients are supported through their cancer diagnostic pathway in a seamless and safe manner.

Provide explanations of procedure and processes to patients and their family regarding the pathway, verbally and by sending out relevant trust leaflets and instruction packs where appropriate.

  • to act as the patients navigator and ensure the management of patients is efficient and effective, taking into account the individual needs of the patients and the requirements of the service, contribute to the achievement of the National Cancer Waiting Time Targets, National Audits and Cancer Registry.
  • Facilitate the patient pathway by co-ordinating and proactively tracking the timeliness of patient treatments within the National guidelines, this includes monitoring patients referred to other Trusts in the Network and influencing their pathway, via the local MDT. The post holder will be responsible for capturing information about patients; ensuring up-to-date relevant information is recorded on the Trusts databases and must also facilitate collection of data for all cancers and enter this onto the Somerset Cancer Registry (SCR).
  • Knowledge and understanding of patient pathways, including the diagnostic and staging investigations and treatment options available, is essential. Excellent organisational communication skills together with high levels of accuracy are also a key requirement for this post.
