Job summary
Due to the retirement of the previous post holder, we have an exciting opportunity within the Worcestershire Countywide Diabetes Nursing Team for a Senior Diabetes Specialist Nurse. This post will be based at Kidderminster Hospital and Treatment Centre, but you may be required to cross cover on other sites from time to time. Diabetes care is in an exciting new phase with technology providing an increasing role in helping people with diabetes to live well with the condition. If you are a motivated enthusiastic nurse with an in-depth clinical knowledge of diabetes, this could be the role for you. Recent experience of managing a caseload of people with diabetes is essential. We will provide a supportive environment with excellent team working so that together we can provide the best care for people living with diabetes. This role is varied as we provide an inpatient service, community service, diabetes structured education and antenatal care. Main duties of the job
Your role in providing diabetes care will include the assessment, planning and implementation of care plans for people living with diabetes in a variety of life stages. This will include providing support to young people as they transition to adult services, support for those in care homes, giving pre-conception advice as well as support during pregnancy.
The team also supports primary care with advice and guidance and complex patient clinics. You will be able to deliver training to other healthcare professionals, students and junior members of the team.
Ensuring that people who live with diabetes have access to structured education and the right technological support is essential. We deliver DAFNE and Xpert programmes and have links with our local Healthy Minds team who are able to provide additional support for people living with long term conditions. You will be expected to support the delivery of Structured Education so having experience of these programmes would be an advantage.
Participation in policy and guideline development and participation in meetings to plan the direction of diabetes care in the county would be welcome and expected of the post holder.

Work both
autonomously and as part of the team to provide a resource of highly specialist
advice, information and support to all members of the primary health care team
in the delivery of effective diabetes care. Act as primary resources to health professional in all care settings giving specialist diabetes advice regarding highly complex symptom management and care planning.
Provide a nurse
prescribing facility to diabetes care patients in line with the agreed
guidelines for Worcestershire Acute Hospitals NHS Trust, following completion
of the accredited university training and registration with the NMC.
Attend diabetes
care MDT meetings, presenting new and complex patients in line with clinical governance.
Provide highly
skilled practical knowledge in the use of complex specialist equipment e.g.:
Continuous Glucose Monitoring and Insulin Pump therapy.
Deliver structured
education for XPERT, XPERT Insulin and DAFNE following accredited training across
Worcestershire.
Participate in developing and providing education/training programmes in the
management of diabetes for patients, carers and healthcare professionals to
improve practice.
Participate in primary research and support clinical practitioners wishing to undertake
research in diabetes care and take an active role in developing and
participation in further research.
As part of a joint commitment to lifelong learning, contribute to the
assessment of personal learning needs and opportunities to inform and develop
personal development plans.
Provide expert leadership and change management skills in developing the diabetes
service.
Function effectively as a clinical nurse specialist in line with the
recommendations of the NMC, DOH.
