Job summary

Worcestershire has an integrated Diabetes Specialist Nursing team who support wide ranging diabetes services. We provide outpatient and inpatient care in 3 hospitals as well as supporting primary care. We have a well-established insulin pump service and are improving access to glucose monitoring with isCGM and rtCGM. The team support diabetes in pregnancy and the multidisciplinary foot service so there always an opportunity to develop an interest in specialist area.

Much of the diabetes care in Worcestershire, including insulin initiation, is provided in primary, care allowing care closer to home. The specialist service supports education and training for healthcare professionals in all roles. Diabetes Structured Education including XPERT programmes and DAFNE are delivered locally by the specialist team and much of this work is delivered virtually.

We are looking to recruit an experienced Diabetes Specialist Nurse to lead the team locally within Kidderminster Treatment Centre. This Team Leader role will be supported by the countywide Lead Nurse for Diabetes and the existing team members. As there is no diabetes consultant presence on site every day, you will need to be a skilled practitioner who is able to work autonomously and have experience in all aspects of diabetes care. With such a variety of aspects of diabetes care on offer, each day will bring a refreshing change.

Main duties of the job

The postholder will deliver diabetes clinical expertise to patients, carers and health care professionals. You will assess and prioritise referrals, managing your case load and supervising the case loads of team members. You will be a facilitator so that more patients have access to and use technology to help them manage their diabetes more effectively. You will be a non-medical prescriber or be actively working towards this qualification.

Participation in multi-disciplinary support sessions at Primary Care Network level and provide support to individual practices with complex diabetes cases is an essential part of the role. As an experienced Diabetes Specialist Practitioner, you will educate and mentor junior members of the team and participate in healthcare professional education across the integrated care system, using a variety of forums to do this.

Whole system working is important, and you will collaborate with other team leaders in order to shape service delivery. You will have a role to play in scoping and delivering quality improvements within the service to enhance the care offered to patients.

Job summary:

To work as a clinical nurse specialist across the accepted domains of clinician, educator, researcher and manager/leader as part of the integrated community and inpatient specialist diabetes care service. The post holder with other key personnel will review, develop and facilitate diabetes care services within Worcestershire. Supporting patients within a variety of care settings as appropriate ensuring a cohesive, holistic and evidence-based approach to the management of diabetes care patients. The role will provide highly specialist diabetes nursing leadership in line with local/national guidelines.

A key function of this post will be to support hospital teams to deliver high quality diabetes care by utilising individual patient review, care planning and staff education.

Key Relationships

All appropriate members of the multidisciplinary health professional teams within the community, hospital, social, nursing, residential homes and voluntary sectors.

Principle responsibilities:

Clinical

  • Work both autonomously and as part of the team to provide a resource of highly specialist advice, information and support to all members of the primary health care team in the delivery of effective diabetes care.
  • Receive referrals from other health professional disciplines and self-referrals from patients and relatives, assess and effectively prioritise these in order to autonomously manage a caseload of patients.
  • Undertake specialist assessments of diabetes care needs and work with primary and secondary care givers to improve the quality of life of patients, relatives and carers living with a chronic long-term conditions.
  • Following specialist assessment, initiates, plans of care for patients with highly complex care needs and responsible for reviewing, evaluating and initiating action within broad policies. Deliver care across all health care settings including patients homes.
  • Frequently uses advanced communication skills to communicate highly sensitive information to patients and carers. E.g., breaking bad news such as foetal abnormalities, loss of sight.
  • Provide a nurse prescribing facility to diabetes care patients in line with the agreed guidelines for Worcestershire Acute Hospitals NHS Trust, following completion of the accredited university training and registration with the NMC.
  • Assess those recently diagnosed as part of the continuing care for families and carers, where specialist intervention in required. Acts on the assessment to plan and implement as appropriate care strategy.
  • Deliver care within Worcestershire to meet the needs of the service.
  • Frequently act as primary resources to health professional in all care settings giving specialist diabetes advice regarding highly complex symptom management and care planning.
