We are looking for a dynamic and enthusiastic pharmacist to come and work alongside the lead pharmacist to deliver best practice care to the patients of Worcestershire, based at the Alexandra hospital, Redditch.

The post offers a skilled clinical pharmacist the opportunity to innovate and support implementation of new ways of working in response to the needs of the acute medicine speciality.

You will have experience of pro-actively influencing prescribing within a secondary care environment, hold or are undertaking a post graduate clinical diploma, be eligible to undertake an independent prescribing course (if not already held) and above all, be a team player!

Main duties of the job

Key responsibilities for the posts will include:

  • Providing a dynamic, proactive pharmacy service to the Acute Medical specialty
  • Providing specialist advice on medications and treatments to the ward and other pharmacists within the trust
  • Identify key areas for service development and expansion in conjunction with the team lead for Acute Medicine
  • Act as a mentor and role model for junior pharmacists rotating into the acute medicine specialty
  • Providing good handover of on-going pharmaceutical care arrangements for patients transferring to medical wards
  • Discharge planning and communication of key issues for patients post discharge across the whole Health Economy

Job responsibilities

The post holder will:

  • Be responsible for delivering the specialist clinical pharmacy and medicines management services to the Acute Medical service at the Alexandra hospital, Redditch.
  • Be prepared to undertake an independent prescribing course or function as an independent prescriber (if qualification already held) within the clinical area as agreed with clinicians and funding, complying with all requirements of the GPhC in relation to registration as an independent prescriber and the associated professional responsibilities.
  • Deliver specialised knowledge of medicines to provide advice to consultants, medical, nursing and pharmacy staff on all aspects of medicines use for patients.
  • Deliver pharmaceutical care to patients; recommend, substantiate and communicate best therapeutic options for patients and implement changes in prescribing practice to ensure evidence-based medicine is followed as appropriate.
  • Undertake risk management for the pharmacy service to allocated area(s) and ensure compliance with medicines legislation, the Trust Medicines Policy, and medicines related national and local guidelines.
  • Undertake clinical audit of medicine use and pharmacy services within the Acute Medicine service as assigned.
  • Provide pharmaceutical teaching to pharmacy, medical and nursing staff.
  • Contribute to keeping drug expenditure within agreed limits as requested by the Director and Deputy Director of Pharmacy.

  • Currently Registered as a pharmacist in UK: Member of the General Pharmaceutical Council.
  • Postgraduate clinical specialist knowledge acquired through diploma level training eg: working towards completion of Diploma in Clinical Pharmacy or equivalent.
  • Currently meeting the RPSGB standard for continuing professional development.
  • Eligible to undertake independent prescribing qualification.

Desirable

  • Accredited independent prescriber.
  • Health Assessment qualification.
  • Working towards RPS Faculty membership.
