Job summary

An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our new Integrated Single Point of Access care navigation. The successful will lead on clinical navigation into Acute Specialities and work with a Multi-Disciplinary team in ensuring patients across the Worcestershire system, receive the right care first time.

You will be joining an established team of affable, dynamic health care practitioners, and nurses who strive to provide the highest level of clinical care in the friendly atmosphere of the acute footprint. Clinical triaging expertise and good communication skills are required as well as excellent time management and prioritization skills is essential for this role.

The post holder will be working autonomously as an ACP, at an advanced clinical level in the direct delivery of high-quality care making critical clinical decisions via telephony and electronic pathways based on advanced clinical skills assessment, working effectively across all four pillars of advanced clinical practice as set out by Health Education England (2017).

The post holder will provide clinical leadership and support within the nursing/AHP teams in developing the service to improve the patient experience, supporting the wider the develop of the Single Point of Access.

You will have access to a wide range on consultants plus access to a large peer group of enthusiastic and high calibre clinicians

Worcestershire Acute Urgent Care teams are dynamic and friendly, and you will work with substantial autonomy, but the support when needed of highly capable colleagues.

The Role

The successful applicant will provide clinical expertise for a wide array of acute injury and illnesses, and provide support to the Single Point of Access triage teams in determining the correct route through which patients can access urgent care.

Alongside the Interface Consultant, they will deliver the call before you convey service and proactively support the remote clinical decision-making of WMAS Crews when attending older people aged 75 years and over. This role will ensure that where possible, people are managed at home rather than through unnecessary conveyance to hospital, and utilise the full breadth and depth of the community directory of service.

Within this function of the role, the successful applicant must be able to prescribe medicines remotely, request and interpret appropriate investigations, and provide adequate safety netting advice for when community care fails and hospital conveyance becomes a necessity.

Hours of work will be discussed and agreed on an individual basis. The breadth of our services and teams means that unusual work patterns can often be accommodated. As well as regular sessions in-hours working within the broader team, we ask that there is a meaningful contribution to our-of-hours patient care (weekends).

Personal Development

There will be a structured developmental support, with an induction, educational agreement, personal development plan and regular appraisals.

Within agreed job plans, 10% of time be available for continuing medical education (CME) and there will be access to the range of Worcestershire Acute Hospital library and postgraduate facilities. In addition, successful applicants will be mentored by an experienced clinician with relevant experience and understanding.

Successful applicants will be able to apply for paid study leave to develop relevant skills. It is expected that they will play a significant role in clinical governance activities and in continual service development to improve efficiency and effectiveness.
