Job summary

An opportunity has become available to work within the trustwide specialist respiratory team as a Pathway Navigator for severe asthma. The team since it's creation has been transformative in the care given to patients with asthma; providing support for those admitted to hospital and the persons caring for them, ensuring the continued safety of patients at hospital discharge and beyond and providing an efficient and supportive service for patients needing a higher level of care. We have an opportunity for a motivated individual to support patients and clinicians in achieving positive outcomes in all aspects of managing asthma. This exciting service area is rapidly expanding, using a growing range of available treatment options and involving national networks to ensure this is achievable. As a Pathway Navigator, your work will be essential to ensuring that patients needing care reach this at the earliest opportunity. You will be supported by a team of clinicians who are passionate and take pride in improving the lives of our patient group, and are committed to increasing the knowledge and expertise of this condition. Administrative and some clinical aspects are combined, making this role unique.

Main duties of the job

To support timely care of persons with severe asthma at local trust level

To support the asthma team to include consultant physicians, advanced clinical practitioners (ACP), clinical nurse specialists (CNS) and other staff in escalating treatments for patients with severe asthma

Manage clinic appointment bookings and communicate with patients regarding care they should expect to receive, to include live bookings during clinic hours

Assist in completing clinical documentation for patients requiring discussion at tertiary centre Multidisciplinary Team Meeting (MDT)

Arrange any necessary investigations for severe asthma patients attending outpatient clinic under direction of the ACP/ CNS team

Ensure the availability of patient information materials to support patient education and care

Occasional participation in basic direct patient care under direction of the ACP/ CNS team

Ensure nationalised and local databases are updated and containing accurate high quality information

Contribute to audit work used to identify current service provision for hospital patients with asthma, used for service improvements

Support registered nurses in the provision of a safe and caring environment

Contribute to the ongoing development of a high quality service for patients with asthma, responding to the specific needs of the population

Key Duties:

Identify hospital inpatients requiring asthma team input on a daily basis, to include the creation of a daily worklist for reviewing clinicians.

Arranging post-discharge remote follow for patients admitted to hospital with asthma

Submitting individual patient details to the data quality team prior to audit of inpatient care to ensure data protection compliance

Completion of the National Respiratory Audit Programme (NRAP) adult asthma workstream, extrapolating information from patient medical notes

Co-ordinate and arrange face to face outpatient appointments for patients requiring input following an unplanned admission or following referral from respiratory consultants and other respiratory teams

Arrange clinical investigations (lung function, blood tests etc) as directed by the ACP/ CNS team, based on patient need, and using agreed referral systems e.g. ICE

Liaise with GP surgeries and other external care providers to obtain necessary patient information (e.g. medication adherence reports) required to escalate their care

Assist in completing required referral forms for patients needing discussion at tertiary centre MDT

Maintain and update patient information to an acceptable standard on the National Severe Asthma Registry via Dendrite clinical systems

Maintain and update local databases containing information regarding patient care

Ensure that patients on biological medication or subcutaneous/ sublingual immunotherapy are re-booked into clinic at appropriate intervals.

Preparation of documents required for planned clinics to include patient questionnaires and clinical notes

Occasional clinical support of patients attending for outpatient appointments to include basic clinical assessment (e.g. blood pressure, weight, peak flow and Fractional Inhaled Nitric Oxide (FeNO) under the direction of the ACP/ CNS team and respiratory physicians

Ensure accurate recording of clinical outcomes and outpatient activity following planned sessions using PAS

To respond to enquiries (telephone, email, internal referral systems, letter and face to face) from patients, carers, primary care providers, tertiary care centres and other sources

Escalating enquiries appropriately and in a timely manner to the correct person(s) and/ or taking any necessary action without reference to ACP/ CNS team if able

To manage patient communications via post, telephone, email and text message

Ensure availability of equipment required for clinical assessment by the ACP/ CNS team to include clinical forms, patient education materials and machine consumables.

Checks and maintenance of specialist equipment used in clinical assessment following training.
