Job summary
An exciting opportunity has arisen for a part time Senior Physiotherapist with specialist skills in assessment and treatment of complex frailty patients and community rehabilitation patients. You will lead the Physiotherapy service across Community based Intermediate Care services including Frailty Virtual Ward, Collaborative Care Team and Reablement services in South East Essex. This post is an excellent opportunity for experienced band 6 Physios looking to develop their management, leadership and service development skills. You will work as part of a large and established multidisciplinary team including, nurses, occupational therapists, HCA's, rehabilitation assistants and Doctors.
Please note due to the community nature of the role, being a car driver and access to a vehicle for work is essential for this post.
Main duties of the job
Provide expert physiotherapy input to patients within intermediate care services including Frailty Virtual Ward, Re-ablement providers, bed-based units and Collaborative Care Team with the aim to facilitate effective prevention of admission to hospital or residential care and encouraging independence and recovery.
- Manage and co-ordinate a caseload of patients with complex conditions, including unstable fractures, complex neurological issues and admission avoidance within intermediate care services.
- To oversee care provision and ensure that accurate records and risk assessment are maintained and regularly reviewed
- To provide expert leadership to Physiotherapy staff through support, supervision and appraisal
- Work in close liaison with Social Services, Hospital Discharge Team, Community Contact Centre, other Rehabilitation providers and Commissioners to achieve and maintain smooth care pathways for patients
- Work in conjunction with the Therapy Lead for Intermediate Care, Intermediate Care Matrons, Head of Intermediate Care and Commissioners on strategic development of IC services in line with future service changes
- Assist in the collation of information to support the evaluation of services once commissioned.
