Job summary
We are seeking a dynamic and innovative individual to support the team to provide holistic and effective nursing care to those people with learning disability requiring the support of Specialist Health Services. In order to support us with improving health outcomes and reducing health inequalities in this service user group.
We have led the way on the development of frailty tools and the introduction of Physical Health Care and Treatment Reviews (PH-CTR) as part of our learning from LeDeR and one of our team members has been nominated for an award for her part in this. The nursing team were also been complimented on their approach and support to people with learning disability during the pandemic
We have also developed a STOMP clinic, led by a Non-Medical Prescriber and Queen's Nurse, this is recognised by NHSE as an example of good practice and the post holder will be offered the opportunity to be part of this.

Main duties of the job
The Community Specialist Health Nurse works with people with a learning disability with additional physical health or mental health needs when they are unable to access mainstream health services, The nurses support other services to make reasonable adjustments so that they are accessible to the service user and work to reduce health inequalities and improve health outcomes.
The team support with desensitisation programmes to reduce health phobias and deliver education and training to other services and carers so they can support the service user group more effectively. The team support other partner agencies to deliver health promotion projects based on learning from LeDeR.
The team also provide Health Coordination for those service users with complex health needs and supports the uptake of Physical Health - Care and Treatment Reviews; using a frailty tool developed by the service.
Annual Health Check training continues to be delivered to GP practices and PCNs and the team work closely with other agencies to improve the uptake and support any follow up required, including the development of Health Action Plans and Hospital Passports.
The team are currently leading on the LD Friendly GP practice accreditation with the support of the local PCNs.
