Job summary

The inpatient wards provide assessment and treatment of service users experiencing a range of mental health problems. We have an exciting opportunity to undertake the role of patient clinical flow coordinator. The post holder will be responsible for leading and facilitating safe and effective discharge in line with the Trust policies and procedures, supporting the planning and co-ordination of patient flow across the care pathways

Main duties of the job

  • The focus of these roles will be to promote patient flow and ensure all admissions remain purposeful.
  • The post holders will be responsible for working with patients and their chosen carers, inpatient and community teams and wider system partners to avoid delayed transfer of care from inpatient services and provide assurance that all necessary actions to resolve ward and trust level delays are in progress. The discharge coordinators will also be responsible for ensuring all system delays are appropriately escalated and agreed actions in progress.

We are looking to expand the clinical and professional expertise within the integrated flow team and will welcome applications from:

Job responsibilities

Work in partnership with the ward teams, external services (e.g., social services, community, and voluntary services), and commissioners to support patient flow from admission to discharge, coordinating seamless and timely packages of care, geared to the needs of the service user.

Support the wider inpatient and community MDT to identify potential barriers to admission/discharge and how these can be resolved at an early stage to prevent unnecessary delays.

Have a working knowledge of the various funding streams and options to support effective discharge and the assessments and processes required to access them.

Effectively analyse the bed capacity using electronic patient information systems, pre-empting emergency and planned admissions to ensure that beds are allocated appropriately, effectively, and in a timely manner to maximise patient care and the patient experience and to ensure national elective admission targets and quality standards are achieved.

Proactively seek out solutions to patient flow issues, challenge practice and escalate deteriorating performance or any concerns at the earliest opportunity.

Connection to community and locality system resource.

Completing documentation regarding funding panels and/or presentation to quality/ funding panels.

Provide a point of contact for patients, relatives and staff on issues relating to patient flow, assistance advice and support.

Be responsible for supporting the assessment, planning, implementation, and evaluation of care within agreed parameters, to ensure meaningful admission and safe, appropriate discharge.

Have a working knowledge of local, national policies and legislation, which govern current service provision and audit.

Be responsible for ensuring effective communication pathways exist.

Ensure that patients, relatives, and carers are given information about discharge planning and have that information explained. Support patients and significant others in their decision making to help promote an ethos of enabling and independence in a timely and consistent manner.

Communicate effectively with patients, families and / or carers regarding the patients admission/discharge plans and inform them of the benefits and risks of suggested options.

Responsible for identifying and contributing to the training needs necessary in providing quality, evidence-based care as appropriate.

Maintain accurate patient records and provide patient information to supervising staff when requested.

Provide visible, local leadership to deliver improved team working and effectiveness, facilitate communication and ensure that high levels of professionalism are always maintained.

Working alongside inpatient teams, ensure Safer Care Bundle principles and NHS Improvement methodology - Red to Green is embedded into practice

Ensuring clinical readiness for discharge principles and escalation with appropriate recording. Comprehensive working knowledge of all individuals on ward and progression towards discharge.

Oversight and escalation of all Ward, Trust and System delays.
