Job summary
The inpatient wards provide assessment and treatment of service users experiencing a range of mental health problems. We have an exciting opportunity to undertake the role of patient clinical flow coordinator. The post holder will be responsible for leading and facilitating safe and effective discharge in line with the Trust policies and procedures, supporting the planning and co-ordination of patient flow across the care pathways
Main duties of the job
- The focus of these roles will be to promote patient flow and ensure all admissions remain purposeful.
- The post holders will be responsible for working with patients and their chosen carers, inpatient and community teams and wider system partners to avoid delayed transfer of care from inpatient services and provide assurance that all necessary actions to resolve ward and trust level delays are in progress. The discharge coordinators will also be responsible for ensuring all system delays are appropriately escalated and agreed actions in progress.
We are looking to expand the clinical and professional expertise within the integrated flow team and will welcome applications from:
- Nursing (Mental Health; General; Learning Disability);
- Occupational Therapy
- Social work.
- Qualified probation officer
- Specialist housing officer
- or equivalent.
