Job summary

An opportunity has arisen within the Colchester Specialist Mental Health Team for a qualified or newly qualified band 5 CPN. the successful candidate will be responsible as the Care Coordinator for the assessment and coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers within a defined catchment area/locality. To provide professional advice and support to other agencies and members of the multidisciplinary team, to act as a role model and resource for junior/less experienced staff.

Main duties of the job

As a Care Coordinator, you will hold a caseload of service users and have a number of responsibilities that include assessment and coordination of care needs. Once needs are identified, you will be expected to plan and implement evidence-based care to the service users and their carers. You will have the support and supervision of your Band 6 colleagues and Team Lead, and as a Band 5 clinician, you will be expected to act as a role model and resource for junior or less experienced staff.

You will be able to draw on the interventions and expertise provided by your MDT colleagues, such as Support Workers, Psychologists, Employment Specialists, Family Therapists and Medical Colleagues.

Assessing new referrals to the team and ascertaining whether they have care needs that need to be addressed within the service
