Job summary

We are seeking enthusiastic Care Support Assistants with excellent communication skills, compassion and empathy to join the Essex Partnership University Foundation Trust NHS family and work as part of the community integrated teams alongside nurses, matrons, occupational therapists and physiotherapists.

Please note for this role you must either drive or have access to a car to fulfil the duties of the role

The teams support the 6 West Essex Primary Care Networks across Epping Forest, Harlow and Uttlesford. Your role as part of the team will be integral in supporting the local population to remain safely at home following discharge from hospital or to prevent them from being admitted to hospital.

You will be visiting people in their own homes to support their recovery and assist with activities of daily living including personal care, supporting with meal preparation, prompting medication and to provide social support such as talking and listening to people and helping them to maintain contact with family and friends.

Main duties of the job

  • To carry out delegated work as set out in the patient's care plan initiated by the coordinator.
  • Support with patients' rehabilitation on returning home from a stay in hospital.
  • To only undertake tasks the post holder feels competent to perform as directed by the Care Manager.
  • To work unsupervised, using own initiative as required.
  • To be adaptable and confident to deal with unexpected situations.
  • To prioritise workload.
  • To act in a professional and confident way at all times.
  • To support patients with all daily living tasks which promote independence and wellbeing.
  • To support patients with food preparation and the monitoring of food and fluid intake asdirected.
  • Adhere to, review and report the effectiveness of the patient's recovery plan andundertaking delegated tasks such as catheter care, stoma care, monitoring of vital signs suchas Blood pressure and simple dressings (once trained and assessed as competent for eachpatient).
  • To assist patients with personal care duties which promote dignity and respect at all times.Assistance with medication as directed.
  • To be able to work autonomously in people's homes, carrying out specified care plan duties.
  • To enable clients to manage their activities of daily living in accordance with their individualcare plans.
  • Participate in appropriate health enhancing activities in order to meet the needs of patients

The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations from across our area. Our aim is to work together in partnership to join up health and care services, improving our populations health and wellbeing and reducing health inequalities. One such example of this collaborative work is the launch of our HCA Academy, an ICS initiative between Essex Partnership University NHS Foundation Trust (EPUT) and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming Health Care assistant (HCA) workforce. The academy models a one workforce cross system approach to the onboarding, induction, education and experience of Healthcare Assistants.

To support this one workforce model the successful candidate for this post will be required to attend our5 day (Monday to Friday) face to face Induction Programme at our MSE ICS Education Hub at Brentwood Community Hospital. Your start date will therefore fall on one of two of the HCA Academy induction dates across the month, which will be discussed and agreed during the recruitment process.

Our HCA Academy Induction programme has been designed to train and support the successful candidate through the early stages of their employment at EPUT and will provide opportunities to enhance skills, knowledge and experience.

Further information will be provided post interview.

This will be followed by an additional 5 day (Monday to Friday) EPUT corporate / mandatory training welcome. (EPUT Welcome will be varied between face to face and virtual delivery).
