37.5 hours per week - negotiable
This is a 1 year fixed term maternity leave cover post for our South Uttlesford PCN dietetic role. This will provide an exciting opportunity to gain experience of working within the South Uttlesford Primary Care Network (PCN), providing a specialised dietetic resource for patients within the PCN. There are 5 GP practices within the PCN with a registered population of circa 51,500.
Main duties of the job
The post holder will be part of the PCN Primary Care Aligned Team (PACT) and will work closely with the other members of the multidisciplinary PACT to support the management of patients with long term conditions.
The post holder will be responsible for providing leadership and dietetic clinical expertise/advice and guidance in partnership with the multidisciplinary PACT. The post holder will fully integrate within the PCN team and become a significant source of expert dietetic advice in the network. The post holder will be clinically responsible for the dietetic assessment and/or treatment of patients with chronic diseases as well as improving care to groups with particular health needs such as diabetes, chronic pain and menopause for example.
