Job summary
The post holder will work as part of the pharmacy and medicines optimisation team, ensuring that all medicines ordered, prescribed and supplied within the locality is carried out in a safe and cost-effective manner. The post holder will work closely with the Locality Care Coordination Pharmacy Technician and Care Co-ordination team, to provide a single joined-up service for people needing health and social care support. To provide specialist clinical pharmacy in a specific area of clinical and/or operational practice, to support the medicines-related care of patients, support the provision and development of medicines management services and to participate in training, education and research as required.1. To actively contribute to, and participate in, the provision and development of pharmaceutical services, and safe and effective use of medicines, within the South Essex Partnership University NHS Foundation Trust, as part of the Trust's pharmacy and medicines management team.2. To participate as an active member of specific ward / unit / clinical teams, as required, and support the safe and effective use of medicines.3. To provide clinical pharmacy and medicines management advice in order to maximise benefit and minimise risk in relation to medicines4. To support and participate in clinical audit.

We are seeking an experienced, enthusiastic, self-motivated pharmacist with excellent communication skills and a clinical background to work within South East Essex integrated health services
The post involves working alongside a multidisciplinary team of healthcare professionals to provide an integrated healthcare service to the community. You will demonstrate a high level of clinical expertise and participate in delivering medicines reconciliation, medication reviews, audits, and other medicines optimisation initiatives.
You will be expected to work independently and show initiative, reporting to the Lead Pharmacist, Community Health Services (South East Essex) and may be required to attend meetings representing the Lead Pharmacist. KEY RESPONSIBILITIESClinical Practice- Conduct clinical medication reviews for patients identified as requiring support- Make appropriate prescribing recommendations and communicate decisions to all stakeholders- Ensure safe and appropriate use of medicines in order to maximise benefits and minimise risk to patients.
