Job summary

  • To work as an autonomous practitioner within the care home team, supporting the delivery of the EPUT vision of patients by caring, learning and empowering care home staff across the MSE delivering evidence base training and education.
  • Liaises with other agencies on the Education, training and competencies of care home staff.

Main duties of the job

To undertake clinical training and practice development within identified care homes. To work in collaboration with Care home managers to deliver training to care home staff including Sepsis identification and the use of Whzan.

To provide professional support and advice to staff in the Care home setting, and to facilitate education and training to registered care homes using evidenced based care.

  • To plan, implement and evaluate Education needs in care homes involving District Nurses, EPUT care home team members and care home staff
  • To develop education packages using research and current clinical evidence with particular emphasis to Sepsis.
  • To actively contribute to the delivery of the NHS and Midlands ambition, to eliminate all avoidable pressure ulcers.
  • To develop effective communication mechanisms between Care home staff and EPUT Care home team, respond as required to warnings raised by Telehealth. Accountability for delivery of service within own caseload.
  • Respond to all urgent clinical issues within care homes as raised by Whzan.
  • To delegate appropriate patients to team members ensuring full utilisation of available skill-mix. To be responsible for effective time management decision-making, organising and planning. Formulate, often-complex work and staff plans

Job description

Job responsibilities

To undertake clinical training and practice development within identified care homes.   • To ensure the effective delivery of the Education agenda for Care homes adhering to Health and Safety regulations.

  • To ensure effective handover of the caseload to another named Nurse during periods of absence.

  • Participate in multi-agency training when required as we move towards greater integrated working partnerships with statutory and voluntary colleagues.

  • Ensure caseload activity is recorded daily and that information department receive activity on all new patient contacts (to assist with forward planning of service provision).

  • To be mentor to pre-registration students and new staff. To educate patients on self-management, independence and health promotion.

  • To participate in clinical audit as identified by the Trust's Nursing Strategy for Clinical Governance.

  • To participate and promote the Trust's Clinical Supervision programme and ensure team members are engaged.

  • To be aware of current research findings and implement evidence based practice in line with the Trust's Nursing Strategy for Clinical Governance.

  • Expected to be aware of your personal responsibilities in the identification and management of all risks, in particular clinical risks. Report near misses and encourage team to report.

  • Maintain safe equipment monitor and record service.

  • Responsible for undertaking risk assessments when ordering specialist equipment.

  • Authorised signature for equipment from stores and clinic stock.

  • Develop IT skills.

  • Act as advocate for patients and carers.

  • To comply with the NMC Code of Professional Conduct.

  • Regularly provide direct emotional support to patient / relatives / carers in distressing circumstances. E.g. terminal illness / palliative care.

  • Regularly provide direct emotional support / supervision to team members when involved in above point.

  • The post-holder will be diverse in their knowledge base in view of increasing roles and responsibilities.
