Advanced Clinical Practitioner - Frailty/End Of Life

Job summary

The Advanced Clinical Practitioner will be an autonomous, accountable practitioner working on their own initiative and in collaboration with medical, nursing, and allied health professional colleagues as required. The post holder will also play a key role within the service / department by supporting specific elements of service improvement, audit and research as well as having an active on-going role in the development, supervision, training and assessment of junior colleagues. The role will be structured around the four core functions of advanced practice:

  • Expert clinical practice within the field
  • Professional leadership and management
  • Education, training and development of self and others
  • Practice and service development, research, and evaluation

The ACP should have a minimum of 15hrs per month (pro rata) for work relating to the non-clinical pillars and to ensure maintenance of competency in all areas of the role within the PCN and Trust wide across EPUT. This will facilitate improvements to patient care through the education of others, and quality/service improvement associated with leadership and research activities.

Main duties of the job

The post holder will work collaboratively within EPUT and North Uttlesford PCN to promote and improve the standards of end of life care and frailty working with the Clinical Lead for End of Life care in EPUT to support Trust wide services. They will be responsible for promoting a shared culture, vision and values to deliver corporate and team objectives in accordance with National Institute for Health and Care Excellence Quality standards for End of Life care and Care of Dying Adults in the Last Days of Life and will lead on the development of systems to ensure there is an effective measurement system to monitor the quality of end of life services.

The post holder will work alongside those working in community health, mental health services and primary care to determine needs of population as well as develop education sessions to develop staff. They will lead on the development of multi-professional education to assist with developing, embedding and sustaining the policies and guidelines associated with end of life
