Job summary

An exciting opportunity has arisen to join the South East Essex Alliance as an Integrated Neighbourhood Team Analyst.

PCN Aligned Community Team (PACT) models have been developed to integrate the delivery of health and care to people with multiple needs, by way of coordinating and aligning services and support that are beneficial to local neighbourhoods and residents. The aim is for multi-agency working to be united at neighbourhood level between primary care, community nursing, mental health, dementia, palliative care, adult social care, care providers and voluntary sector services to provide optimised care.

The PACT model and Integrated Neighbourhood Teams already in place have seen benefits including improving outcomes for patients and reducing hospital readmissions.

Main duties of the job

The post holder will play a key role in supporting the transformation of local services and the development of integrated practice for the benefit of the local population. They will do this by ensuring we take an intelligence led approach to developing, scaling up and refining PACT and Integrated Neighbourhood Teams. You will work towards harnessing local assets and recognising the demographics and characteristics of each local neighbourhood. The capture, analysis and reporting of data will inform service development and design.

With exceptional experience in Information Systems within the NHS, social care and the Voluntary, Community and Social Enterprise Sector, you will be able to identify and extract relevant data, carry out high quality analysis and present information in a professional and accessible way. You will have excellent written and verbal communication skills that enable you to present results and conclusions of analysis to stakeholders within the ICB and wider NHS.
