Job summary
- To work within a multidisciplinary Community Cardiac Service which provides care for people living with heart failure and rehabilitation programmes for people with cardiac conditions.
- To work as part of a team to deliver community based cardiac rehabilitation in a variety of settings and assessment clinic and be flexible to deliver programmes at different times to enable patient choice.
- To act as a skilled autonomous practitioner assessing, managing, planning and delivering appropriate treatment plans tailored to individual needs including care for patients following a cardiac event and those with a diagnosis of heart failure.
- To use evidence based practice to deliver care for the chronic management of heart failure and contribute to develop cardiac rehabilitation services in line with BACPR/NICE guidelines.

- RGN with at least 2 years post registration experience (Band 6) or 3 years (Band 7) or Allied Health Professional with 2-3 years post graduation experience.
- Post registration qualifications in cardiology or cardiac rehabilitation (Band 7)
- Commitment to undertake appropriate education relevant to specialism
Desirable
- Relevant post registration training in Cardiology and/or Heart Failure
- Experience of Cardiac Rehabilitation
- Completion of BACPR courses
- Non-medical Prescriber (Band 7)
