Job summary

The Specialist Community Forensic Team (SCFT) provides a Trust wide service across Essex. We support service users that are currently inpatients in Low & Medium Secure Units across Essex and private facilities to integrate back into the community setting and promoting early discharge from hospital with a high level of intensive support.

This role is to support the MDT with the day to day provision of the occupational therapies ensuring that there is high delivery of care both to the service users we are in-reaching whilst still in the inpatient units and to the service users that we are supporting in the community.

The aim of the service is to promote social inclusion for service users that may have spent many years in inpatient facilities, offering an holistic package that will promote independence and re-integration with the support of the team.

Main duties of the job

The post holder will work as a senior clinician within the Specialist Community Forensic Team providing specialist bio-psycho-social interventions. The post holder will be responsible providing effective leadership and standards of clinical practice and services working closely with the other members of the multi-disciplinary Team. Supervision of junior occupational therapy staff and students will be expected in this role.

The senior clinician will work closely with the partnership agency (Phoenix Futures) that works collaboratively with the delivery of the recovery focused interventions, to promote independence and social integration into the community setting.

About us

The Specialist Community Forensic Team was set up in June 2020 as part of the new models of care to promote early discharge for service users back into the community.

We are now moving through the third stage of our development looking at the through flow and capacity of the team. This involves close working relationships with the local Community Mental Health Teams (CMHT's) across Essex.

We work in partnership with Phoenix Futures a third sector agency who supports with the delivery of the service.

Job responsibilities

Responsibilities within this role will include:

Conduct / contribute to the assessment and treatment process, as well as the observation and ongoing assessment, providing accurate and timely feedback, frequently acting as first point of contact. Utilise specialist assessments when presented with complex cases. Undertake treatment planning, communicating and negotiating treatment plans and clinical reasoning for intervention with service users and their carers.

Communicate agreed treatment/care plans to multi-disciplinary team colleagues and partnership agencies and liaise with them at formal and informal meetings. Participate in the planning, implementation / evaluation and ongoing review of treatment plans. Provide formulation, delivery and review of individualised evidence based care/care packages according to NICE guidance including non-medication strategies particularly for complex cases.

Organise and deliver therapeutic activities for service users in the community setting. Apply a variety of treatment modalities to meet the needs of service users in individual or group format utilising clinical reasoning processes to determine the most appropriate setting for intervention, e.g. home visits.

To manage complex service users within the Specialist Forensic Community Team's caseload. Assessing the needs of the service users response to activities in accordance with agreed models of practice e.g. Model of Creative Ability, MOHO, etc. Provide practical support, advice and education to service users, carers and other disciplines. Provide specialist OT advice on equipment, telecare etc. to other disciplines. Assess service users for access and allocation of equipment within their individual needs. Responsible for the recording of all service user related activity information onto the electronic records system. Produce accurate and timely documentation on all service users in line with your Professional Code of Conduct and Trust policies. Carry out risk assessments including any potential hazards in the service users environment. Attend multi-disciplinary meetings i.e. referral / review / business. Report and document incidents as required.

To liaise with in-patient and community based services to ensure smooth transfer of care through the services.Participate in induction of staff and fieldwork education of students. Facilitate and support in group work such as relaxation, health promotion etc. Undertake carers assessment in line with Trusts key performance indicators. Be competent in use of relevant screening and assessment tools. Provide comprehensive assessment, care planning, treatment and review of care in line with the requirements of Mental Health Care Clusters. Participate in the continuing development of a high quality service that is underpinned by evidence based practice, care pathways / packages, NICE and DH guidance. Participate in research where appropriate and contribute to the development of service policies.
