Job summary

An exciting vacancy has arisen for the position of a Band 5 Community Psychiatric Nurse at the Specialist Dementia and Frailty Service in West Essex. We would welcome applications from both already qualified staff as well as from student nurses close to qualification.

The successful applicant will be working with the existing Multi-Disciplinary Team (MDT) delivering high quality, evidenced based care to patients with Dementia or Older adults with Functional Mental Health Needs across a variety of settings.

We are looking to appoint a dynamic and suitably experienced Mental Health Nurse, who is flexible and able to work on their own initiative, as well as part of a Multi-Disciplinary Team.

Main duties of the job

You will have opportunities to undertake holistic comprehensive assessments, developing MDT patient centred care plans including physical health care, health promotion strategies and social inclusion whilst embracing inter-agency working.

The successful applicant will behonest, competent in prioritising workload in a busy environment and have the ability to problem solve and deal with everyday practice issues.

The successful applicant will receive full 'on the job' training and have opportunities to develop their skills and knowledge in this exciting speciality.

Your working hours will be Monday to Friday 09:00-17:00 hrs, however this will be flexed to meet service and organisation needs.
