This exciting new Consultant Physician post has been developed to provide senior medical oversight and clinical responsibility at EPUT for our newly established Virtual Hospital (VH) and Urgent Community Response Services (UCR). These services are aligned to the West Essex Care Coordination Centre and are an integral part of the West Essex out of hospital care model designed to ensure that patients receive rapid appropriate care and treatment as close to home as possible. Integral to this role is working closely and establishing effective professional relationships with patients and relatives, community hospitals, integrated neighbourhood teams, mental health, acute trusts, EEAST ambulance service, social services and the third sector/voluntary services.
The Virtual Hospitals purpose is to deliver care to residents within the local population of West Essex in the adult's home through a fully integrated community led Virtual Hospital that is capability driven and consultant led with community and acute wrap around service. The remit of the service is to deliver admission avoidance and early discharge. Providing enhanced clinical intervention and monitoring for higher acuity adults that would otherwise need an acute hospital admission or extended stay within acute bedded care, encouraging the adult to be involved in their own care and support their independence in the community.
Main duties of the job
The successful candidate is expected to provide full medical oversight of the clinical caseload, MDT reviews and development of clinical management plans along with providing virtual and face to face new and follow up patient consultations.
You must have full registration with the GMC with a licence to practice in GIM plus another specialty as a consultant (and be on the specialist register held by the RCP)

Job responsibilities
- Diagnosis, Treatment/ management of the presenting clinical condition, including review of any relevant near patient testing (blood and urine results) when conducted.
- Provide advice/ response to any urgent or new emergent clinical issues reported by the Virtual Hospital team.
- Support/ advice for the Virtual Hospital MDT in detection, assessment and management of any presenting conditions including Acute Respiratory Infection, heart failure, Frailty syndromes (falls/mobility decline, acute or chronic confusion (including delirium), bladder and bowel issues, adverse tolerability of prescribed medications.
- Review the medical progress of the patient - including advice/ review of any new patient tests conducted, any clinical/medical concerns arising for patients, including recommending if other tests may be required for current management. Additionally for any medical input into Comprehensive Geriatric Assessment (CGA) and Personal Care and Support Planning (PCSP).
- Senior clinical input into Multidisciplinary care assessment planning based on the framework of Comprehensive Geriatric assessment (CGA) to improve outcomes.
- Support any discussion and engagement with patients/families/carers, including gathering wider collateral information to promote accurate assessment. Input into Advance Care Plans (including exploring personalised goals of care, ceilings of intervention, Treatment Escalation Plan (TEP) PEACE document discussions as well as DNAR resuscitation decisions and anticipatory/active palliative care as clinically appropriate and/or in accordance with personalised expressed preferences on goals of care by the individual.
- Support/Advise on individualised Structured Medicines Review (SMR) in collaboration with the Non-Medical Prescribing (NMP) and pharmacist workforce for the service.
- Liaise with secondary care regarding any planned pathways where necessary can be accessed. For example if further diagnostics are required to support the ongoing management of the patient (eg via planned urgent SDEC or Acute Assessment Unit route or recommendations for elective onward planned secondary care follow up by specialist teams, including memory services)
- Where necessary, provide/arrange face-to-face senior clinical assessment support for the patient where deemed necessary for the patient.
- Assist Virtual Hospital team in planning/setting EDD (exit date) from the virtual hospital service and advise on any appropriate onward referrals to Integrated Neighborhood Teams or Specialist Services to support ongoing management/follow up.
- Maintain of the highest clinical standards in the management of patients.
- Actively participate in both departmental and Trust matters concerning Clinical Governance and Audit.
- Work with the Specialty Management team to ensure that services are delivered efficiently and reflect best practice with processes and governance to match - this is a new role and will involve full delivery of a service and future development.
- Attend meetings as appropriate.
- Fulfil duties of weekly timetable.
- Ensure the provision of good standards of practice and care as described in the GMC Guidance Duties of a Doctor are followed at all times; and that bad practice is reported.
- Ensure that clinical practice is both effective and efficient and that appropriate monitoring of clinical care takes place.
- Provide continuing responsibility for the care of patients under their care and for the proper organisation and functioning of clinical.
- Take responsibility for ensuring that critical clinical incidents, as defined in the Trusts Critical Incident Policy, are reported in a timely manner.
- Participate in yearly appraisal and job planning process.
- Undertake mandatory and statutory training as determined by the Trust.
- Contribute to multidisciplinary teams and lead the co-ordination of patient care.
