Socialprescribing empowers people to take control of their health and wellbeing, focuson 'what matters to me' and take a holistic approach, connecting people to community groups and statutory services for practical and emotional support.
Socialprescribing can help to strengthen community resilience and personal resilience and reduces health inequalities by addressing the wider determinants of health and increasing people's active involvement with their local communities.
It particularly works for people with long-term conditions (including support for mental health), for people who are lonely or isolated, or have complexsocialneeds which affect their wellbeing.
The successful candidate will be able to demonstrate the ability to be a good listener, have time for people and be committed to supporting local communities to care for each other.
You should have experience of working positively with people facing complexsocialand emotional challenges.
You will have great interpersonal skills and a can-do approach to supporting people, community groups and local organisations
Main duties of the job
This post will operate within the Epping North PCN supporting six local GP practices. As part of the PCN's ambitious plans for transforming and developing primary care locally, the post holder will be expected to operate flexibly and collaboratively with all stakeholders. The role will be part of a multi-disciplinary team and will be central to the delivery of key network objectives.
The post will be employed by Essex Partnership University Trust, which is working in partnership with Epping North PCN and is committed to delivering high quality patient care at all times.
The post holder will provide capacity and expertise to support patients in preparing for, or following up, clinical care conversations they have with primary care professionals.
The role will require working closely with GPs and other primary care professionals within the PCN to manage a caseload of identified patients, making sure that appropriate support is available to them and their carers, and ensuring that their changing needs are addressed.
The focus of the role will be to support population health management and to reflect local priorities and health inequalities. The role of the Social Prescriber will support the Clinical Director in coordinating all key activities including access to services, advice, information and that health and care planning is timely. The role will also support digital initiatives and includes supporting the co-ordination of the patient's journey through primary care

- Promote social prescribing, its role in self-management, and the wider determinants of health.
- Build relationships with key staff in GP practices within the local Primary Care Network (PCN), attending relevant meetings, becoming part of the wider network team, giving information and feedback on social prescribing
- Proactively identify and work with a cohort of people to support their personalized care requirements, using the available decision support aids
- Bring together all of the persons identified care and support needs, and explore their options to meet these into a single personalized care and support plan, in line with PCSP best practice.
- Help people to manage their needs, answering their queries and supporting them to make appointments.
- Support people to take up training and employment, and to access appropriate benefits where eligible.
- Raise awareness of shared decision-making and decision support tools, and assist people to be more prepared to have a shared decision-making conversation.
- Ensure that people have good quality information to help them make choices about their care.
- Support people to understand their level of knowledge, skills and confidence (their Activation level) when engaging with their health and wellbeing, including through use of the Patient Activation Measure.
- Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing.
- Explore and assist people to access personal health budgets where appropriate.
- Provide co-ordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers, health and wellbeing coaches and other primary care roles.
