An *EXCITING* opportunity has arisen for an experienced clinician, we are looking for qualified mental health nurses / CPN, for the award winning Op Courage Veterans Integrated Mental health and Well being service. The post will cover East of England and work in collaboration with other NHS providers and charity sector partners,
Main duties of the job
The post holder will be responsible for the care of a caseload of patients under the Op Courage Veterans Mental Health Service that fall within the geographical patch (East of England). This will involve working within the collaborative provision of the Op Courage Veterans Mental Health Service. The role will include responsibility for initial screening, triage, assessment and care planning/risk assessment of clients referred to the service. Liaison with other care providers and charity sector support networks that provide for the Veteran population.
The post holder will work as part of the MDT and will present cases to aid with clinical decision making around treatment, care and support of the Veteran. They will monitor the care and treatment of the Veteran with the aim of working towards discharge from the service.
The post holder will be responsible for referring to and liaising with other agencies involved with the Veteran including locality Mental Health Services, to ensure that all parties are aware of the care and support provided and roles and responsibilities within the plan of care

Job responsibilities
- The post holder will manage a caseload of community based patients.
- Ensure high standards of care are maintained.
- To work as part of the MDT providing triage, assessment, referral and ongoing care for veterans across the area.
- The post holder will present cases to the MDT for clinical decision making and will update the MDT on progress working towards discharge from the service.
- To work as part of the agreed Op Courage partnership and link with other relevant services across the geographical patch.
- To ensure that care plans, risk assessments and all patient care records are completed and maintained in line with set KPIs.
- To provide screening, triage, assessment, implementation and evaluation of plans of care.
- The post holder will work alongside colleagues to provide a 7 day per week duty service to screen and triage new referrals.
- The post holder will be expected to use clinical skills and judgement highlighting and escalating risk. If required they will liaise with and make referral to the clients locality mental health/crisis services.
- Provide information to the Hub Lead in order that they may assess and challenge the service delivery on behalf of the veterans. This is where services are not being agreed according to the veterans mental health agenda across the Midlands and East region.
- Work to improve and promote NHS service provision by actively engaging with the 3rd Sector providers who already have access and are providing support to Veterans across the region.
- Take responsibility for and assist in the management of incidents, ensuring timely investigation and reporting of any incidents.
- Keep up to date with current practices, local and national policies and agendas in relation to Veterans mental health.
- Work to improve and promote NHS service provision by actively engaging with the 3rd Sector providers who already have access and are providing support to Veterans across the region.
- Empower people to address specific needs in relation to health and wellbeing and enable individuals to become experts in managing and taking responsibility for their own condition.
- Maintain up to date knowledge of qualified service providers within the local region.
- In the event of a Veteran being referred to their locality secondary mental health provider (e.g. admission, crisis care or longer term locality community care), the Veterans Liaison Nurse will continue to provide specialist support to the veteran, and will liaise with local services to ensure clarity around roles, responsibilities and treatment within the plan of care.
- The Veterans Liaison Nurse will work alongside psychology, medical input, Veterans Liaison and Support Officers within the service to monitor progress and report to the MDT.
- Update the care/treatment plan and risk/safety plan as required during the veterans care episode.
- Attend regular managerial and clinical supervision.
- Provide managerial/clinical supervision for junior staff.
- Gain consent and agreement around confidentiality/date protection and sharing at the start of the patients journey.
- Contribute to maintaining and improving standards of care.
- Attend relevant training events and conferences relating to Veterans mental health issues.
- Maintain own mandatory training.
- Maintain NMC registration and revalidation.
